目的:观察地佐辛对下肢骨折患者行椎管内麻醉前体位摆放时的镇痛效果。方法:选择80例因下肢骨折拟在椎管内麻醉下行择期手术患者,采用随机、双盲法分为地佐辛组(DEZ组,n=40)和对照组(n=40)。椎管内麻醉体位摆放前15min,DEZ组肌肉注射地佐辛10 mg(2 ml),对照组肌肉注射0.9%氯化钠2 ml。记录2组患者安静平卧位时(T1)、体位摆放时(T2)、腰-硬联合麻醉成功后(T3)的视觉模拟评分(VAS)、数字镇静评分(NSS)、MAP及HR等的改变。结果:组内比较,对照组患者T2时的VAS评分显著高于T1(P〈0.05),DEZ组患者T1、T2时的VAS评分无显著差异(P〉0.05);对照组患者T2时的MAP、HR与T1、T3比较,均显著升高(P〈0.05);DEZ组患者各时点的MAP、HR无明显差异(P〉0.05)。组间比较,T2时DEZ组患者的VAS评分、MAP、HR均显著低于对照组,差异有统计学意义(P〈0.05)。结论:在下肢骨折患者行椎管内麻醉前运用地佐辛,能确切减轻体位摆放时的疼痛,不良反应发生率低,安全可行。
OBJECTIVE To evaluate analgesic effects of dezocineby at putting anesthesia position hefore spinal anesthesia in lower limb fracture surgery. METHODS Eighty patients scheduled for lower limb operation under spinal anesthesia were ran- domly enrolled in this study. Patients were assigned to dezocine group (DEZ group, n = 40) and control group (n = 40). Fif- teen minutes before putting anesthesia position, patients in DEZ group were intramuscularly iniected 10 mg dezoeine (2 ml), while patients in control group were injected with 0. 9% sodium chloride soIution(2 ml). Visual analog scale (VAS), numeric sedation scale (NSS), MAP and HR were recorded at quiet supine position (TI), after placed in anesthesia position (T2), and after spinal anesthesia (T3). RESULTS By anesthesia position (T2), VAS scores, MAP and HR in control group increased remarkably (P〈0. 05), while no significant change was observed in DEZ group (P〉0. 05). VAN scores, MAP and HR by T2 were significantly lower in DEZ group than those in control group (P〈0. 05). CONCLUSION In patients with lower extremi- ty fractures, dezocine has notable analgesic effects by putting anesthesia position before spinal anesthesia.
