目的:观察分析CD7+的急性单核细胞白血病的骨髓象特征。方法:结合细胞形态学、细胞化学染色和免疫分型的方法,回顾82例急性单核细胞白血病患者的骨髓象,分析CD7+急性单核细胞白血病患者的骨髓象特征。结果:82例急性单核细胞白细胞患者中,18例表达CD7抗原,占22.0%。CD7+急性单核细胞白血病患者的骨髓增生程度偏低,巨核细胞数量偏少,白血病细胞过氧化物酶染色(POX)阳性率偏低,差异有统计学意义(P〈0.05)。CD7+急性单核细胞白血病原始细胞胞核折叠明显,胞浆量较少。结论:与CD7-的急性单核细胞白血病相比,CD7+急性单核细胞白血病细胞更加原始,形态上具有T淋巴细胞的部分特征。
Objective:To investigate the characteristics of the bone marrow features in CD7 positive acute monocytic leukemia (CD7+-AML).Methods:A total of 82 cases of acute monocytic leukemia were reviewed.The characteristics of the bone marrow cell morphology in CD7 positive acute monocytic leukemia were analyzed,combined with the methods of cell morphology,cell chemical staining and immunophenotype.Results:Among 82 cases of acute monocytic leukemia,18 cases expressed CD7 antigen,accounting for 22.0%.It was found that in CD7+ acute monocytic leukemia the proliferation of bone marrow was reduced and the amount of megakaryocytes was lessened.Positive rate of peroxidase staining (POX staining) was significantly lower.These differences had statistical significance (P0.05).The nucleus in leukemia cells of CD7+ acute monocytic leukemia were more folded than those in the cells of CD7-acute monocytic leukemia.Meanwhile,the amount of cytoplasm was also lessening.Conclusion:Compared with CD7-acute monocytic leukemia,leukemia cells of CD7+ acute monocytic leukemia not only have more primitive features,but also combine a part of characteristics of T-lymphocytes.
