Ultraviolet A Enhances Cathepsin L Expression and Activity via JNK Pathway in Human Dermal Fibroblasts

目的:研究特应性皮炎(AD)患者正常及皮损部位酸/碱缓冲能力差异,及修复皮肤屏障后对正常皮肤及皮损部位皮肤酸/碱缓冲能力的影响。方法:招募30名AD患者,在皮损部位皮肤及正常部位皮肤检测pH值、皮肤屏障指标(TEWL值及皮肤含水量),并用0.1mol/L酸/碱刺激+wash法检测皮损及正常皮肤的酸/碱缓冲能力。随后用在AD皮损表面使用5%的L-乳酸凝胶4周调节皮损部位pH值,检测皮损屏障修复情况及皮损酸/碱缓冲能力恢复情况。结果:30名AD患者严重度SCARD评分为(8.75±3.5)分,平均年龄(18±5.2)岁。pH值检测结果发现,AD患者的正常皮肤pH值为(4.7±0.61),皮损部位pH值为(6.5±0.37)(P=0.03)。皮肤屏障检测结果发现,AD皮损处含水量降低、TEWL值升高。酸/碱缓冲能力检测发现,AD皮损对酸/碱缓冲能力下降。在AD皮损表面使用5%的L-乳酸凝胶调节皮肤pH值后,发现AD患者皮损pH值恢复能力上调,同时TEWL值较未使用部位降低、含水量升高。结论:特应性皮炎患者皮损部位酸碱缓冲能力下降、皮肤屏障受损。修复AD皮损缓冲能力、调节皮损部位pH值,可修复特应性皮炎皮损部位皮肤的皮肤屏障。

英文摘要:

Objective To investigate the role of acid/base buffering capacity in atopic dermatitis(AD) pathogenesis especially in skin barrier damaging and repairing. Methods 30 atopic dermatitis patients were recruited.Skin moisture pH and TEWL value were detected on lesions and normal areas of AD patients.Acid/base buffering capacity was also tested via 0.1mol/L acid/base irradiated + washed method.Changes of skin pH value,acid/base buffering capacity and skin barrier was tested after using 5%L-lactic acid gel for 4 weeks on AD lesion. Results pH value of the 30 AD patients, average age(18 ± 5.2)and SCARD score(8.75 ± 3.5),was(4.7 ± 0.61)on normal skin while(6.5 ± 0.37)on skin lesions(P=0.03).Skin barrier detection showed that the skin moisture of AD lesions was decreased comparing to the normal skin area while the TEWL value was increased.Skin buffering capacity and self-correction recovery ability of stratum corneum and dermis was decreased in AD lesions.4 weeks after using 5%L-lactate gel on the surface of AD skin lesions, the buffering capacity and self-correction recovery ability of stratum corneum and dermis was better than that of the unused gel lesions.At the same time, the pH value and TEWL value was lower than that of the unused gel lesions while the skin moisture was higher. Conclusion The buffering capacity and skin barrier of atopic dermatitis patient lesions were damaged.Adjusting buffer capacity and the pH value to normal level can repair skin barrier of atopic dermatitis skin lesions.
