The ＂ Hong Kong model＂ of general practice is one of the successful models in the world. They are an important part of this modellts, including its publie medical policy, ＂ network system integration services＂ , appointment clinic system and classification system, training of family doctors, management of chronic diseases, clinical management system developed, reasonable clinical audit, the use of community resources and strengthening patient self-management. This model of our inspiration is mainly reflected in the following aspects, including optimization of the management team of chronic diseases, professional transformation of general practitioners, improve the performance appraisal system, improve and strictly implement the classification system and strengthen the information construction, and so on.
