目的:评价猪冠状动脉前降支注入不同浓度微球栓塞后对左室收缩和舒张同步性的影响。方法:小型猪10只,经心导管在冠状动脉前降支注入5万、12万、15万个微栓塞球。分别在基础状态和微栓塞后2h、6h、1周的静息状态以及负荷状态下行常规超声心动图和组织多普勒检查;以声诺维(SonVue)行实时心肌声学造影(real-time myocardial contrast echocardio-graphy,RT-MCE)。分别测量每次超声检查时M型超声左室二尖瓣和乳头肌水平的前壁增厚率、前壁收缩达峰时间、下壁增厚率、下壁收缩达峰时间、脉冲组织多普勒前壁收缩开始时间、前壁收缩达峰时间、前壁收缩期积分、前壁舒张开始时间、下壁收缩开始时间、下壁收缩达峰时间、下壁收缩期积分、下壁舒张开始时间和左室射血分数;采用声学密度(acoustic density,AD)测定功能测量RT-MCE左室短轴二尖瓣水平前壁和下壁的心肌血流量(myocardial blood flow,MBF)以及乳头肌水平前壁和下壁的MBF。结果:前降支不同程度微栓塞猪模型超声心动图各个指标在基础状态、微栓塞后2h、6h和1周的静息状态及负荷状态下均无显著差异(P〉0.05)。结论:猪冠状动脉前降支注射微球栓塞时,如微球剂量较小,造成栓塞面积较小,未能明显影响心肌局部血流灌注时,不会影响心肌收缩和舒张的同步性。
Objective:To evaluate the impact of different microembolization on cardiac systolic and diastolic synchronism in pigs.Methods: Ten miniature pigs underwent microembolization procedure with 50,120,150 thousands microsphere injection through anterior descending coronary artery.Routine echocardiography and tissue doppler imaging were performed at baseline,2 hours,6 hours and 1 week after microembolization at resting and stress states.Real-time myocardial contrast echocardiographies(RT-MCE) were done using SonoVue.The anterior wall thickening,time-to-peak anterior wall contraction,posterior wall thickening,time-to-peak posterior wall contraction in M-mode imaging,and anterior wall contraction starting time,time-to-peak anterior wall contraction,anterior wall systolic integral,anterior wall dilatation starting time,inferior wall contraction starting time,time-to-peak inferior wall contraction,inferior wall systolic integral,inferior wall dilatation starting time in tissue Doppler imaging both at mitral valve level and papillary muscle level at left ventricle short axis views were measured respectively as well as left ventricle ejection fraction.The myocardial blood flow(MBF) of anterior wall and inferior wall at mitral valve level and papillary muscle level at left ventricle short axis views were detected using acoustic density(AD).Results:The difference among all the index in miniature pigs with different anterior descending coronary artery microembolization at baseline,2 hours,6 hours and 1 week after microembolization at resting and stress states had no any statistical significance(P0.05).Conclusions: The systolic and diastolic index of left ventricle don't changed as well as systolic and diastolic synchronism in miniature pigs underwent microembolization procedure when the dosage of microsphere is small and infarct sizes are not large.The systolic and diastolic synchronism will not be affected when MBF is not reduced obviously.
