目的:评价血清中可溶性趋化因子配体16(CXCL16)水平在克罗恩病(CD)与溃疡性结肠炎(UC)中的诊断价值以及与其他炎性标志物、临床疾病活动度、内镜下疾病活动度的关系。方法:收集了2011年武汉大学中南医院消化内科确诊为炎症性肠病(IBD)病例55例,其中UC 29例,CD 24例,纳入年龄和性别匹配的55例正常人作为对照。内镜下对疾病程度进行评分,运用ELISA方法检测血清中CXCL16水平。结果:UC和CD患者血清中CXCL16水平分别为(1.99±0.47)ng/ml和(2.24±0.74)ng/ml,均较正常对照[(1.54±0.34)ng/ml]明显升高(P〈0.05);血清CXCL16水平与CDAI评分(r=0.588,P〈0.05)和CRP(r=0.427,P〈0.05)均显著相关,CXCL16对炎症性肠病具有一定诊断学价值[CD曲线下面积为0.841(95%CI,0.741-0.941);UC曲线下面积为0.792(95%CI,0.698-0.887),P〈0.01],但是特异性较低,尤其在UC中更明显。结论:CXCL16可作为IBD血清学分子标志物并用于对IBD诊断的辅助工作。
Objective:To assess the level of soluble chemokine ligand 16(CXCL16)in Crohn's disease(CD)and ulcerative colitis(UC),and to study its diagnostic value,and its correlation with other inflammatory markers,clinical disease activity and endoscopic disease activity.Methods:Fiftythree confirmed cases of inflammatory bowel disease(IBD)(29UC and 24CD)and 55 age and sex matched healthy controls were recruited for the study.CXCL16 level in the sera were quantitatively measured by using a commercial enzyme-linked immunosorbent assay.Endoscopic evaluation of the intestine was done in most of the IBD patients.Statistical evaluation of data was done,using the SPSS17 software.T-tests,receiver operating characteristic(ROC)curve analysis,and Pearson's correlation test were done.Results:CXCL16level in UC([1.99±0.47]ng/ml),and in CD([2.24±0.74]ng/ml)were both significantly higher(P〈0.05)than in control group([1.54±0.34]ng/ml).Only in CD,CXCL16 was significantly correlated to CDAI(r=0.588,P〈0.05),and CRP(r=0.427,P〈0.05).No significant correlation between CXCL16 and endoscopic scores(SES-CD,Rachmilewitz score in UC)were identified in either CD or UC.CXCL16 was found to have diagnostic value(AUC:0.841[0.741-0.941]in CD versus 0.792[0.698-0.887]in UC,P〈0.001).However,the specificity was low,especially in UC.Conclusion:CXCL16is a serological biomarker of IBD,which can be used as an adjunct diagnostic tool for the diagnosis of IBD.
