目的 通过经腹主动脉腔内灌注高浓度弹力蛋白酶建立兔腹主动脉瘤(AAA)模型,观察不同灌注保持时间对兔AAA成瘤率及死亡率的影响。方法 24只新西兰雄兔随机分为A组、B组、C组各8只,每组均经双腔导管注入10μl、浓度为100 U/ml的弹力蛋白酶,灌注保持时间A组3-5 min,B组7-10 min,C组20-30 min;后用生理盐水冲洗管腔3次;分别于术前及术后第7天行腹主动脉彩超检查,术后第7天处死动物,行组织学病理染色观察血管灌注部位组织学变化。结果 A组存活8只,灌注局部血管扩张率40.3%-63.1%,其中5只扩张率大于50%,达到AAA成瘤标准,成瘤率62.5%,术后7 d存活率100%;B组存活8只,局部血管扩张率87.5%-109.2%,均达到成瘤标准,成瘤率100%,术后7 d存活率100%;C组存活3只,灌注局部血管扩张率237.7%-252.0%,成瘤率100%,术后7 d存活率为37.5%。结论 高浓度弹力蛋白酶制作兔AAA模型,灌注时间保持7-10 min具有成瘤率高、死亡率低、操作简易等优点,易于推广。
Objective To establish the rabbit abdominal aortic aneurysm model by abdominal aortic perfusion with high concentration, and observe the effect of different perfusion time on the abdominal aorta aneurysms formation rate and mortality in rabbits. Methods 24 male New Zealand male rabbits were randomly divided into three groups ( group A, B, C), with 8 rabbits in each group. Using a double- lumen catheter, 10μl of 100 U/ml elastase was perfused, into the abdominal aorta in each group, perfusion retention time were: group A 3 to 5 minutes, group B 7 tol0 minutes and 20 to 30 minutes in group C, and then the abdominal aorta was flushed three times with normal saline. Doppler ultrasound examinations of the abdominal aorta were performed before surgery and on day 7 after surgery. The rabbits were sac- rificed on day 7 after surgery, and the perfused segment of the abdominal aorta was observed visually and after staining. Results 8 rabbits in group A survived and the perfusion segment dilation rate was 40.3% - 63.1%, the dilation rate was greater than 50% in 5 rabbits, which satisfied the criterion of aneurysm formation in the abdominal aorta. Therefore, the aneurysm formation rate of group C was 62.5%, the survival rate was 100% on day 7 after surgery. 8 rabbits survived in group B; the perfusion segment dilation rate was 87.5% - 109.2%, the aneurysm formation rate and survival rate were 100%. In group C, 3 rabbits survived, and the perfusion segment dilation rate was 23% 7% - 252.0%, with an aneurysm formation rate of 100%. However, the survival rate was only 37.5% on day 7 after surgery. Conclusions Abdominal aortic perfusion of high concentrations of elastase , perfusion retention time 7 to 10 minutes is an easy and feasible method to induce AAAs in rabbits with a high aneurysm formation rate. low death rate, and short perfusion time.
