目的探讨正电子发射断层显像(PET)/CT评价吡格列酮抗动脉粥样硬化斑块作用的可行性。方法将20只雄性新西兰大白兔随机分为药物干预组(n=10)和对照组(n=10)。采用主动脉球囊拉伤术与间断高脂饲料饲养的方法建立动脉粥样硬化模型,并进行药物诱发斑块破裂实验。药物干预组在整个实验过程中于原饲料里添加吡格列酮10mg/(kg·d)。分别于实验中期(第8周)和晚期(第16周)进行PET/CT扫描,自动测量感兴趣区的最大标准化摄取值(SVUmax)和平均标准化摄取值(SVUmean)。斑块破裂诱发实验后对2组兔主动脉进行解剖并留取组织进行病理学分析。结果药物干预组的血栓动脉段明显低于对照组(14.6%vs 39.1%,P=0.000),斑块激发实验后破裂斑块的SUVmean(1.486±0.486 vs 0.655±0.235,P=0.000)和SUVmax(1.862±0.564 vs 0.843±0.058,P=0.000)均明显高于非破裂斑块。药物干预组斑块面积、巨噬细胞计数、新生血管计数较对照组明显降低(P〈0.05)。SUVmean和SUVmax与斑块面积、巨噬细胞计数呈明显正相关,但与新生血管计数无明显相关(P〉0.05)。结论 PET/CT作为一种无创的功能-形态多模式成像技术,能够有效监测吡格列酮抗动脉粥样硬化作用。
Objective To study the role of PET/CT in assessing the anti-atherosclerotic properties of pioglitazone.Methods Twenty male New Zealand white rabbits were randomly divided into pioglitazone treatment group(group A,n=10)and control group(n=10).A rabbit atherosclerosis model was established by aorta sprain procedure and intermittent high-cholesterol feeding.Plaque rupture was induced with drugs.Pioglitazone(10 mg·kg-1·d-1)was added into the diet for group A.The animals underwent PET/CT scanning at week 8and week 16 respectively.The SUVmeanand SUVmaxin the region of interest(ROI)were measured.After plaque rupture inducing test,aortic tissue samples were taken for histopathological analysis.Results The incidence of aorta thrombus was significantly lower in group A than in control group(14.6% vs 39.1%,P=0.000).The SUVmeanand SUVmax were significantly higher in ruptured plaques than in non-ruptured plaques.The area of plaques was significantly smaller and the number of macrophagocytes and new blood vessels was significantly lower in group A than in control group.The SUVmeanand SUVmax were positively related with the area of plaques and the number of macrophagocytes and negatively related with the number of new blood vessels.Conclusion PET/CT play an important role in assessing the anti-atherosclerotic properties of pioglitazone.
