目的探讨原发性闭角型青光眼小梁切除早期滤过泡形态与24h眼压的相关性。方法选择原发性慢性闭角型青光眼患者138例,均采用小梁切除术治疗,术后1个月对滤过泡形态进行评价,检测24h眼压。结果不同滤过泡高度、范围、血管分布、渗漏试验等级的24h眼压波动值差异有统计学意义(P〈0.05)。微囊组与对照组24h眼压波动值的均值、最大值、最小值、差值比较,差异有统计学意义(P〈0.05)。单因素回归分析,术前眼压、滤过泡高度、滤过泡范围、滤过泡血管分布及滤过泡微囊结构与术后24h眼压波动值相关性有统计学意义(P〈0.05)。多因素回归分析,滤过泡高度、滤过泡范围分级及滤过泡微囊结构是术后24h眼压波动值的主要相关因素(P〈0.05)。结论原发性闭角型青光眼小梁切除早期滤过泡形态与术后24h眼压有关,滤过泡高度、滤过泡范围及滤过泡微囊结构对24h眼压的影响显著。
Objective To investigate the primary angle-closure glaucoma trabeculectomy blebs early morphological correlation with 24 h IOP. Methods The primary chronic angle-closure glaucoma patients 138 cases, were used trabeculectomy, after a month on bleb morphology evaluation, 24 h intraocular pressure was tested. Results Bleb height, extent, vascularity, leak test level fluctuations in the value 24 h IOP difference were statistically significant (P〈0.05). 24 h microcapsule group and control group the mean IOP fluctuations in value, maximum value, minimum value, the difference, the difference were statistically significant (P〈0.05). Correlation of univariate regression analysis, preoperative IOP, bleb height, bleb range bleb vascularity and bleb microcapsule structure and postoperative 24 h intraocular pressure fluctuations in the value were statistically significant (P〈0.05). Multivariate regression analysis, bleb height, bleb bleb scope and structure of microcapsules and postoperative IOP fluctuation value 24 h, the main underlying factor were negatively correlated (P〈0.05). Conclusion Primary angle-closure glaucoma trabeculectomy blebs morphology of early postoperative IOP 24 h relate bleb height, bleb area and bleb microcapsule structure 24 h IOP significantly affected.
