目的:探讨瞬时波强(WI)评价高血压不同构型心室功能的价值。方法:测量105例高血压组和98例对照组颈总动脉WI,左心室结构和功能指标。结果:①从对照组,高血压正常构型组,向心性重构组,向心性肥厚组到离心性肥厚组:其血压水平,左室舒张末内径,重量指数递增;中层短轴缩率递减;左室射血分数在向心性肥厚组增大,离心性肥厚组减低;左室早期充盈压递增(P〈0.05)。②高血压组瞬时加速波强(W1)高于对照组,减速度波强(W2)低于对照组(P〈0.05)。③与正常构型和向心性重构组比较,W1在向心性肥厚组增大,离心性肥厚组降低;W2在向心性肥厚组和离心性肥厚组降低(P〈0.05)。结论:WI可以作为无创评价高血压不同构型左心室功能的简便有效的方法。
Objective:To evaluate wave intensity(WI) on left ventricular(LV) performance in the different hypertensive remolding hearts.Methods:105 hypertensive and 98 control subjects were underwent noninvasive evaluation of carotid arterial wave intensity,LV structure and function.Results:①There were increasing trends in the levels of blood pressure,LV end-diastolic diameter and LV mass index in the control,normal geometry group,concentric remodeling group,concentric and eccentric hypertrophy group.LV ejection fraction increased in the concentric hypertrophy group and decreased in the eccentric hypertrophy group in which mid-wall fractional shortening showed a decreasing trend.LV diastolic filling pressure presented increased progression accompanied by LV remodeling(P0.05).②Transient acceleration wave intensity(W1) in hypertensive subjects were higher than that in the control(P0.05).Transient deceleration wave intensity (W2) was lower than that in the control(P0.05).③W1 in the concentric hypertrophy group was higher and lower in the eccentric hypertrophy,compared with that in the control group,normal geometry group and concentric remodeling group(P0.05).W2 was lower in concentric hypertrophy group and eccentric hypertrophy group than that in the control,normal geometry group and concentric remodeling group(P0.05).Conclusion:WI is a noninvasively obtained,clinically useful parameter for evaluation of LV performance.
