目的:观察单纯性肥胖儿童中脂肪肝的发生情况,以及血脂、转氨酶等指标的改变,并探究炎性因子在肥胖脂肪肝发生中的作用。方法:将121例单纯性肥胖儿童根据影像学检查结果分为脂肪肝组和非脂肪肝组,并抽取31例健康儿童为对照组。测量所有儿童的身高、体重、腰围、臀围,计算BMI值、腰臀比、腰围身高比。检测血脂,转氨酶及hs-CRP,脂联素,TNF-α的水平。结果:三组儿童的BMI、腰围、腰臀比均有显著性差异(P〈0.05),脂肪肝组及非脂肪肝组儿童腰围/身高比(WHtR)与对照组相比有显著性差异(P〈0.05),脂肪肝组及非脂肪肝组儿童TG及TCH与对照组比较有显著性差异(P〈0.05);三组儿童的AST及ALT均有显著性差异(P〈0.05)。结论:谷丙及谷草转氨酶在肥胖脂肪肝早期会显著升高,脂肪肝形成与炎症反应增强有关。
Objective:To observe changes of blood lipids,transaminase and other related parameters,and to investigate actions of inflammatory factors on the occurrence of nonalcoholic fatty liver in obese children.Methods:A total of 121 obese children and 31 healthycontrols were enrolled in this study.Obesity was defined as a body mass index exceeding the 95 thpercentile for the patients' age and sex.Nonalcoholic fatty liver was diagnosed via ultrasonographic examin-ation.Obese children were divided into two groups,fatty liver group(C group)and nonfatty liver group(B group),31 healthy children as control group(A group).The height,weight,BMI,waistline,hipline,waist-to-hip ratio and Waist to height ratio were measured and documented for every child.Blood lipids,transaminase,hs-CRP,adiponectin and TNF-α were also measured.Results:There are significant differences in BMI,waistline and waist-to-hip ratio between any two groups(P〈0.05).Waist to height ratio,TG and TCH was different significantly between B/A,and C/A(P〈0.05).AST and ALT were also found different significantly between any two groups(P〈0.05),and levels of transaminases increased gradually among A,B and C groups.Conclusion:In this study,levels of AST and ALT were found higher than controls in early phase of nonalcoholic fatty liver in obese children.Enhancement of inflammatory reaction was in relation with fatty liver.
