选取16头健康小型猪随机分为A、B、C、D 4组,分别以0 mm Hg、10 mm Hg、12 mm Hg、15 mm Hg的气腹压进行充气腹试验,通过监测脉搏血氧饱和度、呼吸末二氧化碳分压及气道峰压来探讨不同CO_2气腹压力对小型猪呼吸系统的影响。结果显示,A组试验猪脉搏血氧饱和度、呼吸末二氧化碳分压及气道峰压均保持平稳,B、C、D组试验猪脉搏血氧饱和度呈下降趋势,呼吸末二氧化碳分压及气道峰压呈上升趋势。本试验确定了不同CO_2气腹压对小型猪呼吸系统的影响程度是D组〉C组〉B组。表明在小型猪长时间腹腔镜手术充气腹时,为获得足够术野和操作空间的同时,应尽量降低气腹本身对机体呼吸系统的影响,选用10-12 mm Hg的气腹压值为宜,不应超过15 mm Hg。
Sixteen healthy minipigs were randomly divided into four groups,which were named A,B,C and D.Carbon dioxide pneumperitoneum was established at 0,10,12,15 mm Hg,respectively to explore the different effects on respiratory system by monitoring the saturation of pulse oximetry,end-tidal carbon dioxide pressure,and peak airway pressure. The result showed that CO_2 pneumperitoneum in group B,C and D caused a decrease of SpO_2 and increase of Pet CO_2 and Ppeak. The experiment determined that the effects of CO_2 pneumperitoneum on respiratory system were group DCB.Thus,the pneumperitoneum pressure should be set less than 15 mm Hg and better within 10-12mm Hg,which would cause less impact on respiratory system and gain enough operating space at the same time.
