目的调查蒙古族农牧民C反应蛋白(CRP)水平和代谢综合征(MS)组分及其聚集性之间的关联性。方法采用现况研究的方法对内蒙古通辽市农牧区2 536名20岁以上的蒙古族居民进行生活方式危险因素、体质、身高、腰围、臀围、血压、空腹血糖、血脂和CRP的调查和检测。对蒙古族农牧区居民CRP水平与MS组分聚集作用的患病危险性进行分析,采用有序多分类的非条件Logistic回归分析模型,计算出MS组分的不同暴露组合下的患病危险性,即OR值及其95%CI。结果代谢综合征年龄性别标化患病率是17.1%,男性(11.4%)低于女性(21.6%)(P〈0.01)。按CRP四等分位划分为4个等级,同时根据代谢综合征组分聚集的个数分组(0,1,2,3,4,5个代谢综合征组分)。在5个单因素及5个多因素Logistic回归模型中,4级CRP水平与代谢综合征每个组分均有关联(均P〈0.01)。有序多分类Logistic回归分析结果表明:调整性别、年龄、总胆固醇、低密度脂蛋白胆固醇后,3级和4级CRP水平与聚集一个及一个以上代谢综合征组分的危险性是1级的2.35(1.91~2.89)和6.87(5.50~8.57)倍。代谢综合征组分积累(增加一个或一个以上组分)的危险性随着CRP等级的增加而增加(趋势性检验,P〈0.01)。结论 CRP水平增加和代谢综合征单个组分及其组分的聚集性有连续、独立的显著关联。
Objective To investigate the association between C-reactive protein(CRP) concentrations and individual metabolic syndrome(MS) components and their accumulation in the farmer and pas- turing areas in Inner Mongolia,China.Methods A cross-sectional survey was conducted in Chaoluto country and Guribanhua country of Tongliao City of Inner Mongolia form June 2003 to August 2004.All the survey and examination were completed in a total of 2 536 participants aged 20 or more(1 038 men and 1 498 women).Lifestyle risk factor were investigated,and body weight,height,waistline,hipline, blood pressure etc.measured,and fasting plasma glucose,blood lipids and CRP for all participants.Results Standardized prevalence rate of MS was 17.1%,it was lower in male(11.4%) than that in female (21.6%)(P0.01).Concentrations of CRP in each one of the individual MS components population were higher than those without corresponding individual MS components(P 0.01).In the 5 univariate and 5 multivariate Logistic regression models for individual MS components,grade 4 of CRP was strongly associated with every MS component(all P≤0.01 ).The geometric mean CRP for those with 0, 1,2,3,4,and 5 components of the metabolic syndrome were 1.56,1.66,1.91,2.16,2.44,and 2. 74 mg/L,respectively(P0.01).The result of ordinal Logistic regression analysis showed that after adjustment for gender,age,TC,LDL-C,the risk of grade 3 and grade 4 of CRP accumulating one or one more MS components were 2.35(1.91~2.89)and 6.87(5.50~8.57) times the lowest quartile(grade 1) of CRP.The risk of MS components accumulation(one or one more MS components addition) was increasing with the increasing of CRP grades(P0.01).Conclusion A significant,continuous,and dependent association existed between increased concentrations of CRP,individual MS components and MS components accumulation in Inner Mongolia people,China.
