目的 探讨在支气管肺发育不良患儿中转化生长因子β1蛋白的表达变化及其意义.方法 收集北京军区总医院附属八一儿童医院早产新生儿重症监护室患儿血清共65例,其中有支气管肺发育不良趋势患儿34例(血清支气管肺发育不良组),血清对照组31例;同时收集家属放弃后死亡的早产儿肺组织6例,肺组织支气管肺发育不良组和肺组织对照组各3例.采用酶联免疫吸附试验方法 检测血清中转化生长因子β1蛋白的浓度;苏木素-伊红染色观察肺组织病理变化,免疫组化及荧光技术检测转化生长因子β1蛋白的表达变化.结果 转化生长因子β1蛋白血清支气管肺发育不良组表达量(9 819.1±5 540.1pg/mL)远高于血清对照组(5 952.7±2 666.0pg/mL),两组间比较差异有统计学意义(t=3.634,P=0.001);免疫组化结果 显示肺组织支气管肺发育不良组染色面积及强度均大于肺组织对照组,应用专业图像软件分析显示肺组织支气管发育不良组平均光密度值(0.245±0.152)高于肺组织对照组(0.116±0.073),两组比较差异有统计学意义(t=-2.282,P=0.042);免疫荧光显示肺组织支气管肺发育不良组肺泡上皮细胞内转化生长因子β1荧光表达强,密度大且饱满;肺组织对照组荧光表达弱,呈空泡状.结论 转化生长因子β1的过度表达可能在支气管肺发育不良患儿的肺发育障碍及肺间质纤维化中发挥重要作用.
Objective To investigate expression of transforming growth factor-β1 ( TGF-β1 ) in premature infants with bronchopulmonary dysplasia and its significance. Methods 65 serum samples, including 34 serum samples from infants with BPD trend( serum-BPD group, SBPD group in brief) and 31 serum samples from neonates in the control group( S-control group in brief) in preterm NICU in our hospital were taken. At the same time, 6 specimens of lung issues of dead and parents-abandoned premature infants with or without BPD after obtaining informed consent by parents, including 3 dead premature infants due to BPD (lung tissues BPD group, T-BPD group in brief) and 3 dead premature infants due to other causes ( lung tissues control group, T-control group) were taken for study. ABC-ELISA was used to detect serum TGF-β1 protein concentration of the infants, HE staining was used to observe pathological changes of the lung tissues and immunohistochemistry and immunofluorescence staining were used to detect expression level of TGF-β1 protein. Results The expression quantity of TGF-β1 in S-BPD group(9 819.1 ± 5 540.1 pg/mL) was significantly higher than S-control group (5 952.7 ± 2 666.0pg/mL) and the difference was significant( t = 3. 634 ,P =0. 001 ). The immunohistochemical analysis results showed coloring area and intensity of lung tissues in T-BPD group were greater than T-control group, IPP image analysis system showed that mean optical density( mean density) in T-BPD group (0.245 ± 0.152) was significantly higher than T-control group ( 0.116 ± 0. 073 ) and the difference was significant ( t = - 2. 282 ,P = 0. 042 ). lmmunofluorescence test showed that fluorescence of lung alveolar epithelial cells in T-BPD group was higher ( with full and optical density of fluorescence) than that in T-control group ( with weaker expression of fluorescence and showing vacuoles ).
