[目的]通过Meta分析比较前交叉韧带(anterior cruciate ligament,ACL)损伤自体韧带重建残端保留与不保留临床疗效的差异。[方法]检索并收集2000年1月-2016年10月公开发表在Pubmed、Cochrane图书馆、Springer Link、万方数据库、中国知网(CNKI)以及中文科技期刊全文数据库(VIP)关于交叉韧带损伤自体韧带重建残端保留与不保留临床疗效比较的文章,严格按照纳入和排除标准及文献质量评分标准收集数据,运用Review Manager 5.3统计学软件对所提取的文献数据进行meta分析。[结果]共纳入15篇文献。Meta分析显示:前交叉韧带损伤自体韧带重建残端保留术对患者膝关节的客观稳定性KT-1000/2000测量[MD=-0.21,95%CI(-0.25,-0.16),P〈0.01]、早期本体感觉恢复[MD=-0.72,95%CI(-1.16,-0.28),P〈0.01]、后期本体感觉恢复[MD=-0.45,95%CI(-0.66,-0.25),P〈0.01]明显好于非保留组;主观满意度Lysholm评分[MD=0.20,95%CI(-0.13,0.53),P=0.24]、Cyclops病发生率[RR=1.12,95%CI(0.36,3.52),P=0.84]、手术时间[MD=-4.49,95%CI(-38.85,29.88),P=0.80]两种手术差异有统计学意义。[结论]前交叉韧带损伤自体韧带重建残端保留术对患者膝关节的客观稳定性,早、远期本体感觉恢复有明显的促进作用;保留与不保留术对患者的主观满意度、Cyclops病发生率、手术时间没有明显影响。
[Objective] To compare the clinical outcomes of remnant preserving versus removing during anterior cruciate ligament reconstruction. [Methods] Published studies about the clinical results of remnant preserving versus removing techinques in anterior cruciate ligament reconstruction during January 2000 to October 2016 were searched on the Pubmed, Cochrane Library, Springer Link, Wanfang, CNKI and VIP database. After methodological quality of the trials was critically assessed, a meta analysis was conducted using Revman 5.3 software. [Results] A total of 15 trials meeting criteria were identified from the literature review. Meta-analysis revealed a significant difference in the KT-1000/2000 [MD=-0.21, 95%CI(-0.25,-0.16), P〈0.01], the early proprioceptive recovery [MD =-0.72,95%CI(-1.16,-0.28),P〈0.01], the post-proprioceptive recovery [MD=-0.45, 95%CI(-0.66,-0.25), P〈0.01] between the remnant preserving and removing ACL reconstructions, the former was superior to the latter. However, there were no significant difference in the Lysholm scores [MD=0.20, 95%CI(-0.13, 0.53), P=0.24], the incidence of cyclops lesions [RR=1.12, 95%CI(0.36, 3.52), P=0.84] and the average operation time [MD=-4.49, 95%CI(-38.85,29.88), P= 0.80] between the two techniques. [Conclusions] The remnant preserving technique in anterior cruciate ligament reconstruction plays a positive role in promoting proprioceptive recovery and enhancing the objective stability of the knee. The subjective satisfaction, the incidence of cyclops lesions and the average operation time are not correlated to whether or not remnant preserving.
