目的探讨血清脂蛋白(a)[Lp(a)]、载脂蛋白A1(ApoA1)、载脂蛋白B(ApoB)与脑梗死的相关性。方法收集2015年1月至2016年3月在重庆医科大学附属第一医院住院的脑梗死患者207例作为脑梗死组,再按性别将脑梗死组分为男性脑梗死组和女性脑梗死组。另选择同期在该院住院的非脑梗死患者207例作为对照组。对比分析脑梗死组、男性脑梗死组、女性脑梗死组与对照组Lp(a)、ApoA1、ApoB与脑梗死的相关性。结果 (1)与对照组比较,脑梗死组Lp(a)及ApoB水平较高、ApoA1水平较低,男性脑梗死组Lp(a)、ApoB水平较高,女性脑梗死组Lp(a)水平较高,且差异均有统计学意义(P〈0.05)。(2)多因素分析显示,Lp(a)与脑梗死相关(OR=1.715,P=0.001),且在男性脑梗死及女性脑梗死均存在相关性(OR=1.615、2.079,P=0.017、0.011)。结论 Lp(a)是脑梗死的独立危险因素,且对于女性更为明显。ApoA1可能是脑梗死的保护因素,ApoB可能是脑梗死的危险因素,ApoA1及ApoB与脑梗死的相关性可能存在性别差异。
Objective To investigate the correlation between serum lipoprotein(a)[Lp(a)],apolipoproteinA1 (ApoA1) and apolipoprotein B (ApoB) with cerebral infarction. Methods Two hundred and seven inpatients with cerebral infarction in our hospital from January 2015 to March 2016 were recruited as cerebral infarction group and divided into the male cerebral in- farction group and female cerebral infarction group according to gender. At the same time other 207 inpatients with non-cerebral infarction in this hospital were enrolled as the control group. By comparing the control group with the cerebral infarction group, male cerebral infarction group and female cerebral infarction group, the relationship between Lp (a), ApoA1 and ApoB with cere- bral infarction was analyzed. Results (1)Compared with the control group, the cerebral infarction group had a significantly higher level of Lp (a) and ApoB (P〈0.05),lower level of ApoA1 (P〈0.05), the male cerebral infarction group had higher level of Lp (a) and ApoB (P〈0.05), and the female cerebral infarction group had higher level of Lp (a), the differences were statistically significant(P〈0.05 ). (2)The multivariable analysis results showed that the level of Lp (a) had significant correlation with cere- bral infarction ( OR = 1.715, P=0.001 ). The level of Lp ( a ) was correlated with male cerebral infarction ( OR = 1.615, P=0.017 ) and female cerebral infarction(OR=2.079 ,P=0.011 ). Conclusion Lp(a) is an independent risk factor for cerebral infarction,which is more obvious in female. ApoA1 may be a protective factor for cerebral infarction,while ApoB may be a risk factor for cerebral infarction, and the correlation between ApoA1 and ApoB with cerebral infarction may have sex difference.
