目的探讨肺腺癌组织学亚型与18F-脱氧葡萄糖(FDG)摄取及表皮生长因子受体(EGFR)突变状态的关系。方法回顾性分析2013年1月至2014年10月94例术前行PET/CT检查肺腺癌患者,根据最新国际肺癌研究学会/美国胸科学会/欧洲呼吸病学会的多学科分类方法对每例腺癌组织学亚型进行评估。并检测EGFR基因突变状态,对可能影响肿瘤EGFR突变的临床病理特征进行单因素分析和多因素回归分析。结果全组EGFR的突变率为45.7%(43/94),其中女性(58.7%)、不吸烟(63.6%)、最大标准化摄取值(SUVmax)≤3(65.2%)和组织学亚型评分较低者具有更高的突变率。EGFR突变型患者SUVmax低于EGFR野生型患者(P=0.018),组织学亚型评分较低者SUVmax低于评分较高者(P〈0.001)。多因素logistic回归分析表明组织学亚型评分是EGFR突变的独立影响因素(P=0.033)。结论肺腺癌新的病理学分类与SUVmax及EGFR突变状态有良好的相关性,腺癌组织学亚型的构成是EGFR突变的关键因素。
Objective To investigate the correlation of histologic subtypes with 18F-FDG uptake and EGFR mutation status in lung adenocareinoma. Methods From Jan 2013 to Oct 2014, a total of 94 patients with adenocareinoma who under- went 18F-FDG PET/CT scan before surgical resection were retrospectively reviewed, each ease was classified according to the new IASLC/ATS/ERS classification system, and evaluated by a scoring system. The EGFR mutation status of each case was analyzed, and clinicopathological factors of those may affect EGFR mutation status were evaluated by univariate and multivariate analysis. Results The frequency of EGFR mutation was 45.7% (43/94), women(58.7% ), nonsmokers(63.6% ), maxi- mum standardized uptake value(SUVmax) ≤3 ( 65.2% ) and lower histologic subtype score were found to be significantly asso- ciated with higher incidence of EGFR mutations. SUVmax of adenocarcinoma with mutant EGFR was significantly lower than that with wild-type EGFR( P = 0. 018 ), SUVmax of adenocarcinoma with lower histologic subtypes score was significantly lower than that with higher histologic subtypes score(P 〈 0. 001 ). Histologic subtypes score was found to be independent risk factor for EGFR mutation according to multivariate logistic regression analysis(P = 0. 033 ). Conclusion The stratification for lung based on IASLC/ATS/ERS classification system is correlated with SUVmax and EGFR mutation status, the components of primary tumor based on histologic subtypes are crucial for EGFR mutation status. words]
