目的研究关节腔注射Nell-1对于骨关节炎状态下的颞下颌关节软骨是否有保护作用。方法对24只新西兰白兔行双侧颞下颌关节盘部分切除术建立骨关节炎模型。4周后,于所有白兔右侧关节注射Nell-1作为实验组,左侧关节注射同剂量生理盐水作为对照组。分别于注射后12周和24周各随机处死12只白兔,通过组织学观察和逆转录酶-聚合酶链式反应(RT-PCR)检测评价Nell-1对关节软骨的保护作用。结果所有白兔的颞下颌关节均出现了骨关节炎样的组织学病变,但在实验选择的时间节点上,实验组关节软骨的病变程度较对照组轻微,注射后12周实验组Ⅱ型胶原与蛋白聚糖的mRNA表达明显高于对照组,但注射后24周实验组与对照组在蛋白聚糖及白介素1的mRNA表达上无明显差异。结论本研究表明关节腔注射Nell-1或可成为治疗颞下颌关节骨关节炎中软骨退变的一种新选择。
Objective To study whether Nell-1 injection into articular cavity has any protection effect on the temporomandibular joint cartilage with osteoarthritis. Methods Carry out bilateral temporomandibular joint disc excision on 24 New Zealand white rabbits to establish the osteoarthritis models. After 4 weeks, inject Nell-1 in right side joints of all rabbits as the experimental group, and inject normal saline of same dosage in left side joints of all rabbits as the control group. 12 and 24 weeks after the injection, kill 12 rabbits at random successively, through histology observation and RT-PCR test to evaluate the protection effect of Nell-1 on the joint cartilage. Results Histologic lesion of osteoarthritis occurs to the temporomandibular joints of all rabbits; however, the lesion degree of joint cartilage of the experimental group was slighter compared with that of the control group, mRNA expressions of collagen II and proteoglycan of the experimental group were obviously higher that of control group 12 weeks after the injection; however,mRNA expressions of proteoglycan and of interleukin 1 of experimental group and control group did not have any difference 24 weeks after the injection. Conclusion This study indicates that Nell-1 injection into articular cavity may be a new selection to treat cartilage degeneration of temporomandibular joint osteoarthritis.
