[目的]调查福建省三明市三元区并殖吸虫的传播媒介、传染源、传播途径、易感人群以及流行因素.[方法]对采集螺、蟹标本,作并殖吸虫尾蚴及囊蚴的检查与观察.以并殖吸虫成虫抗原作皮内试验作为人群感染的过筛,阳性者以ELISA法检测抗体.[结果]检查第1中间宿主有放逸短沟蜷、洱海螺、拟钉螺两种(待定种)和沼螺,仅在洱海螺中检及1只阳性.第2中间宿主福建华溪蟹和三角肢华南溪蟹感染率为44.3%(62/140),检出斯氏狸殖吸虫等5种并殖吸虫囊蚴.3份野猫粪便2份检出大量并殖吸虫虫卵.人群皮试阳性率8.6%(21/243),血清抗体阳性率23.8%(5/21);男性感染率较女性高;青少年组感染率最高;生吃或半生吃溪蟹的人群感染率比未食者高.三元区为并殖吸虫各阶段宿主的孳生提供了适宜的地理环境.[结论]三元区是一个以斯氏狸殖吸虫为主的并殖吸虫病高度流行疫源地,青少年、男性以及生吃或半生吃溪蟹史的人群易受感染.
[Objective]To study the epidemiology and epidemic factors of Paragonimusin in Sanyuan, Sanming, Fujian. [Methods] The metacercariae and cercariae in snails and crabs were examined. Inira-dermal test with adult worms antigen was used for screening people infection, and the antibody was detected by using ELISA for the positive.[Results] Semisulcospira liberlina and other snails were examined and cercariae was found only in Tricularsp. nov. The infection rate of Sinoptamon fujianense and Huananpotamonsp. nov (the second intermediate host) was 44.3% (62/140), and 5 species metacercariae of Paragonimus ( P. skrjabini et al ) were found. A lot of eggs of Paragonimus in dejecta of 2 cats were found from 3 wild cats; The positive rate with skin test was 8.6% (21/243) in residents and antibody positive rate was 23.8% (5/21) ; Infection rate in male was higher than that of female and the highest rate was in teenagers. Infection rate who ate crabs (raw or half-cooked) was higher than one who didn't. There are suitable environment in Sanyuan for Paragonimus breeding. [Conclusion] Sanyuan district is a endemic area of Paragonimiasis as the most of P. skrjabini. Male teenagers and raw or half-cooked crabs eating are easy to be infected by Paragonimus.
