目的测量OA滑液中IL-1Ra和IL-1β水平,并评估它们的水平或它们之间的比率与OA临床症状之间的相关性。方法按美国风湿病学会诊断标准纳入48例膝OA病人,利用酶联免疫吸附试验(ELISA)测定滑液中IL-1Ra和IL-1β水平,按Kellgren-Lawrence分级评估影像学表现,按加拿大西安大略和麦克玛斯特大学骨关节炎指数(WOMAC)评估整体临床症状。结果 OA滑液中IL-1β水平为1.1(0.1~12.28)pg/ml,IL-1Ra水平为2380(209~17111)pg/ ml,IL-1Ra-1β比率为2484(279~7000)。两种细胞因子或其比率与OA临床指标之间不存在任何相关性,但两种细胞因子之间存在较强的相关性(rs=0.9123,P〈0.001)。结论 OA滑液中IL-Ra/IL-1β比率与OA临床症状之间不存在相关性,但远远高于正常滑液中这两种细胞因子的比率。尽管如此,软骨降解还是会发生。因此,提高局部IL-1Ra水平的治疗策略可能无法从根本上逆转OA病变进展。
Objective To measure the levels of IL-1Ra and IL-1βin the synovial fluid of the patients with osteoarthritis (OA)and to assess the correlation between the level of IL-1Ra or the level of IL-1β,or IL-1Ra/IL-1βratio and OA clinical symptoms.Methods Forty-eight patients with knee OA according to the classification of the American College of Rheumatology were selected for the study.The concentrations of IL-1Ra and IL-1βin the synovial fluid were measured by enzyme-linked immunosorbent assay(ELISA).Kellgren-Lawrence grade was used to assess the radiological grade and Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)was used to score clinical symptoms of the patients,respectively.Results In the synovial fluid of OA patients,the concentration of IL-1βor IL-1Ra was 1.1(0.1-12.28)pg/ml or 2380(209-17111)pg/ ml,respectively,the IL-1Ra/IL-1βratio was 2484(279-7000).Although IL-1Ra or IL-1βlevel or the ratio of the two cytokines was not correlated with OA clinical symptom,there was significantly high correlation between the two cytokines levels(rs= 0.9123.P0.001).Conclusions The IL-Ra/IL-1βratio in knee OA is not correlated with OA clinical symptom,but it is much higher than that in normal synovial fluid.Even the balance inclined to IL-1Ra,the articular cartilage deterioration can still occur. So the therapeutic strategy of increasing local IL-1Ra level can not reverse the progression of OA essentially.
