目的探讨胺碘酮对人心房超快激活延迟整流钾电流(IKur)的作用。方法取择期行心脏外科手术的8例房颤患者和10例窦性心律患者的右心耳,分离心房肌细胞,膜片钳记录IKur。另取部分细胞随机分为4组:空白对照组、胺碘酮组、三碘甲状腺原氨酸(T3)组、T3+胺碘酮组,24 h后测定IKur。结果 IKur在电压钳制于+60mV时出现峰值电流。心房肌细胞IKur峰值密度窦性心律患者与房颤患者比较无统计学差异(P〉0.05);胺碘酮组与空白对照组比较无统计学差异(P〉0.05),T3组显著高于空白对照组(P〈0.05),T3+胺碘酮组显著低于T3组但高于空白对照组与胺碘酮组(P均〈0.05)。结论胺碘酮本身并不影响IKur,但可拮抗T3对IKur的升高作用。
Objective To investigate the effect of amiodarone on human atrial ultra-rapid delayed rectifier potassium current(Ikur).Methods Right atrial appendages were obtained from 8 patients with atrial fibrillation and 10 patients with sinus rhythm.Atrial myocytes were isolated and IKur currents were recorded by patch-clamp.Then atrial myocytes were divided into four groups: control,amiodarone,triiodothyronine(T3),T3 plus amiodarone groups.After 24 hours,IKur currents were detected by patch-clamp.Results At +60 mV clamped voltage the peak of IKur current appeared.The peak current density of the IKur current in the sinus rhythm group had no significant difference with that in the atrial fibrillation group.No significant difference in the peak current density was seen in the control group and the amiodarone group.However,the peak current density in the T3 group was significantly higher than that in the control group(P〈0.05).The peak current density in T3 plus amiodarone group was significantly lower than that in the T3 group(P〈0.05) and was still higher than that in the control group and the amiodarone group(P〈0.05).Conclusions Amiodarone could not directly affect IKur current,but could inhibit the IKur current's upregulation by triiodothyronine.
