目的 检测丙酮酸脱氢酶激酶4(pyruvate dehydrogenase kinase 4,PDK4)在结肠癌组织及癌旁黏膜组织中的表达情况,初步探讨其临床意义。方法 应用组织芯片技术结合免疫组织化学染色方法,检测本院收治的172例结肠癌患者术后组织标本及86例癌旁肠黏膜组织中PDK4蛋白的表达,对结果及临床病理学参数进行统计学分析。结果 PDK4在172例结肠癌组织中高表达率显著高于86例癌旁肠黏膜组织(57.0%vs 26.7%,P〈0.01),PDK4蛋白表达高低与结肠癌的分化程度、临床分期、淋巴结转移、预后等密切相关(P〈0.05),且PDK4高表达组患者的生存时间显著低于低表达组患者(P〈0.01)。在临床分期低的结肠癌患者中,PDK高表达者预后较差(P〈0.01);而在临床分期高的结肠癌患者中,PDK表达高低预后差异无统计学意义。结论 PDK4蛋白在结肠癌组织中表达高于癌旁肠黏膜组织,其可能参与结肠癌的发生、发展。
Objective To determine the expression of pyruvate dehydrogenase kinase 4 (PDK4) in colon cancer and adjacent mucosa tissues, and explore its clinical significance. Methods Tissue microarray and immunohistochemical staining were applied for the detection of PDK4 expression in the colon cancer samples from 172 colon cancer patients and adjacent mucosa tissues from 86 cases. The results and clinical pathological parameters were analyzed for statistical significance. Results The expression rate of PHD4 protein was significantly higher in the 172 colon cancer tissues than the 82 adjacent mucosa tissues(57.0% vs 26.7%, P 〈 0.01). And its expression was closely associated with tumor differentiation, clinical stage, lymph node metastasis and prognosis ( P 〈 0.05 ). The patients with higher expression level of PDK4 had shorter survival time than those with lower expression (P 〈 0.01 ). Among the patients at lower clinical stage, those with higher PDK4 expression level suggested a poorer prognosis ( P 〈 0. 01 ). But no such phenomena were seen in the patients at advanced stage of colon cancer. Conclusion The expression of PDK4 protein is higher in colon cancer tissues than adjacent mucosa tissues, and the protein may participate in the occurrence and development of colon cancer.
