目的探讨终末期肾病(ESRD)患者腹膜透析(PD)和血液透析(HD)对自然杀伤(NK)细胞和T细胞亚群的影响。方法 2013年1月至12月诊治的25例ESRD患者中PD组13例,HD组12例。分别比较两组患者透析前、透析后6个月及12个月时,患者NK细胞和T细胞亚群的变化。结果 (1)PD治疗组:PD后6、12个月CD3+、CD4+、CD4/CD8较透析前均升高(P均〈0.05);PD后12个月CD3+、CD4+均高于PD后6个月(P均〈0.05),但CD4/CD8无明显变化(P〉0.05)。PD后6个月与透析前、PD后12个月相比,CD8+无统计学差异(P〉0.05)。NK细胞(CD16+/CD56+)百分比在PD后6、12个月较透析前均降低(P均〈0.05),且PD后12个月低于PD后6个月(P〈0.05)。(2)HD治疗组:HD后6、12个月CD3+、CD4+、CD4/CD8与透析前相比均升高(P均〈0.05),CD8+无变化(P〉0.05);HD后12个月与HD后6个月相比CD3+下降(P〈0.05),CD4+、CD4/CD8无变化(P均〉0.05);NK细胞百分比在HD后6个月与透析前相比无差异(P〉0.05);HD后12个月较透析前及HD后6个月均降低(P均〈0.05)。结论 PD治疗对ESRD患者T细胞免疫功能的抑制低于HD治疗,两种透析方法对NK细胞活性的降低无差别。
Objective To investigate the influences of peritoneal dialysis (PD) versus hemodialysis (HD) on natural kil- ler (NK) cells and T cell subsets in patients with end-stage renal disease (ESRD). Methods From January 2013 to De- cember 2013 ,twenty-five patients with ESRD were divided into PD group (n = 13 ) and HD group (n = 12) according to dialysis methods. The changes of NK cells and T cell subsets before dialysis and 6,12 months after dialysis were compared. Results In PD group, CD3 + , CD4 + , CD4/CD8 at 6 and 12 months after PD raised significantly compared with pre-dialy- sis ( all P 〈 0.05 ) ; CD3 + , CD4 + at 12 months after PD were higher than those at 6 months after PD ( all P 〈 0.05 ), but CD4/CD8 remained unchanged (P 〉0. 05);the CD8 ~ among pre-dialysis, 6 and 12 months after dialysis were similar ( P 〉 0.05 ). The percentages of NK cells ( CD16 +/CD56 + ) at 6 and 12 months after PD decreased compared with pre-di- alysis ( all P 〈 0.05 ), and it of 12 months after dialysis was lower than that of 6 months ( P 〈 0.05 ). In HD group, CD3 + , CD4+ , CIM/CD8 at 6 and 12 months after HD raised significantly compared with pre-dialysis ( all P 〈 0.05 ) , while CD8 + remained unchanged ( P 〉 0. 05 ). Compared with 6 months after HD, CD3 ~ of 12 months after HD decreased ( P 〈 O. 05 ) , while CD4+ , CIM/CD8 remained unehanged( all P 〉 0.05 ). The percentage of NK cells at 6 months after dialysis was sim- ilar to pre-HD ( P 〉0. 05 ) ,but it of 12 months after HD was lower than those of pre-dialysis and 6 months after HD ( all P 〈0. 05 ). Conclusion The suppressive effect of PD on T cell immunologic functions is lower than that of HD, while PD and HD induced lowering of NK cells activity is similar in ESRD patients.
