常规超声、超声弹性成像及二者联合鉴别诊断乳腺肿块良恶性的效果分析

目的:探讨常规超声、超声弹性成像(UE)及二者联合在鉴别乳腺肿块良恶性中的应用价值。方法对125个乳腺肿块的常规超声特点、弹性成像图进行分析,并且和病理金标准对照,通过受试者工作特征(ROC)曲线下面积的比较,对二者单独及联合应用鉴别乳腺肿块的效果分别进行评估。结果常规超声、超声弹性成像及二者联合诊断乳腺肿块良恶性的结果与病理检查的符合率为77.6%、84.8%和92.0%;3种检查方法诊断乳腺恶性肿块的符合率分别为61.9%、76.2%、90.4%。二者联合鉴别乳腺肿块良恶性的 ROC曲线下面积(0.947)大于单独常规超声(0.843)(Z =2.57,P =0.01)、超声弹性成像(0.899)(Z=1.52,P =0.12)。结论常规超声与弹性成像联合应用可有效提高乳腺癌的诊断符合率。

英文摘要:

Objective To explore the value of general ultrasonography,ultrasonic elastography (UE)and their combined application in differential diagnosis of breast masses.Methods The characteristics by the general ultrasonic and UE in 125 cases of breast masses were analyzed and compared with gold standard of pathology;the areas under the receiver operating characteristic (ROC)curves were compared to estimate the diagnostic effects of general ultrasonic,UE and their combination respectively.Results The respective pathological coincidence rates by general ultrasonic,UE and their combined application in differentiating benign and malignant breast tumors were 77.6%,84.8%,92.0%;these three methods had coincidence of 61.9%,76.2%,90.4% in differentiating the malignant breast tumors.The areas under the ROC curve in differentiating the benign and malignant of breast masses by the combined application of general ultra-sonography and UE (0.947)were bigger than those of general ultrasonography (0.843)(Z=2.57,P =0.01)and UE (0.899)(Z=1.52,P=0.12).Conclusion Combined application of general ultrasonography and UE can improve the diagnostic coincidence rate of breast cancers obviously.
