目的:探讨糖尿病与良性前列腺增生(BPH)发生和进展的相关性。方法:对临床明确诊断BPH 190例患者的临床资料进行分组对比分析。结果:190例患者中,单纯BPH组100例,BPH合并糖尿病者90例(47.4%)。BPH合并糖尿病组患者的国际前列腺症状评分(IPSS)明显大于单纯BPH组[(23.72±6.08)分比(21.49±7.83)分,P〈0.05],最大尿流率(Qmax)明显小于单纯BPH组[(8.0±4.6)ml/s比(9.9±5.3)ml/s,P〈0.05]。在BPH患者中,空腹血糖异常组患者的IPSS[(24.07±4.73)分比(22.34±5.12)分]、前列腺体积[(75.41±58.36)ml比(72.04±40.49)ml]明显大于空腹血糖正常组(P〈0.05);而餐后正常血糖组与餐后异常血糖组各指标比较均无显著性差异(P均〉0.05)。与单纯BPH组相比,BPH合并糖尿病组的不同糖尿病病程患者的BPH各指标无显著性差异(P均〉0.05)。结论:良性前列腺增生同时合并糖尿病多见;糖尿病尤其异常空腹血糖水平可促进良性前列腺增生的发生以及进展。
Objective:To study relationship between diabetes mellitus(DM) and development of benign prostatic hyperplasia(BPH).Methods:Clinical data of 190 patients diagnosed as BPH were grouped,compared and analyzed.Results:Among 190 BPH patients,90 cases(47.4%) were complicated with DM(BPH+DM group).International prostate symptom score(IPSS) of BPH +DM group was significantly higher than that of pure BPH group [(23.72±6.08) scores vs.(21.49±7.83) scores,P〈0.05],maximum urinary flow rate(Q_max) of BPH+DM group was significantly lower than that of pure BPH group [(8.0±4.6)ml/s vs.(9.9±5.3) ml/s,P〈0.05].Among BPH patients,IPSS [(24.07±4.73) scores vs.(22.34±5.12) scores] and prostate volume [(75.41±58.36) ml vs.(72.04±40.49) ml] of abnormal fasting blood glucose(FBG) group were significantly higher than those of normal FBG group(P〈0.05);There were no significant difference between normal and abnormal postprandial blood glucose groups in each index(P〉0.05 all).Compared with clinical indexes of pure BPH group,BPH indexes of BPH+DM patients with different DM course of disease showed no significant difference(P〉0.05 all).Conclusion:Patients with benign prostatic hyperplasia often complicate with diabetes mellitus;diabetes mellitus,especially abnormal level of fasting blood glucose,can promote occurrence and development of benign prostatic hyperplasia.
