目的探讨T细胞免疫球蛋白黏蛋白结构域分子3(Tim-3)mRNA表达在异基因造血干细胞移植(allo—HSCT)后急性移植物抗宿主病(aGVHD)中的作用。方法回顾性分析100例allo—HSCT患者,根据患者aGVHD状态[0(未发生)~I度aGVHD组、Ⅱ。Ⅳ度aGVHD治疗前组、Ⅱ~Ⅳ度aGVHD治疗好转组]及移植后不同阶段(+14+30d、+31-+60d、+61~+100d)比较各组外周血单个核细胞Tim-3mRNA及血浆IFN-У、IL-2水平。结果0~I度aGVHD患者+31-+60dTim-3mRNA相对定量(7.24±2.79)明显高于+14~+30d(4.60+1.66)及+61~+100d(3.86±1.36)(P值均〈0.05);+14~+30d、+31-+60d、+61-+100dII~IV度aGVHD治疗前组的Tim-3mRNA水平分别为9.54±3.05、10.14±3.28、12.82±4.20+1±30d、+61±100d,均高于0~I度aGVHD组及治疗好转组;+31-+60dⅡ-IV度aGVHD治疗前组明显高于治疗好转组(2.49±0.89),与0~I度aGVHD组(7.24±2.79)相比差异无统计学意义;11~Ⅳ度aGVHD组中Tim一3mRNA水平在不同aGVHD程度、不同脏器之间比较差异均无统计学意义(P〉0.05)。Ⅱ-Ⅳ度aGVHD治疗前组血浆IFN-1、IL-2水平明显高于其他各组,治疗好转组血浆IFN小IL一2水平与0~I度aGVHD组比较差异无统计学意义。结论Tim-3在aGVHD的发生过程中发挥着重要作用。
Objective To investigate the role of Tim-3 gene expression in acute graft-versus-host disease (aGVHD) after allogeneic hematopoietic stem cell transplantation (allo-HSCT). Methods Patients after allo-HSCT were retrospectively analyzed, this cohort of patients were divided into different groups according to disease states (grade 0- I aGVHD group, grade II- IV aGVHD group, improved aGVHD group) and time periods (+ 14-+30 d, +31-+60 d, +61-+ 100 d) to compare PBMC Tim-3 mRNA expression and plasma IFN-y, IL-2 concentrations among them. Results RT-PCR showed that in grade 0- I aGVHD group, Tim- 3 mRNA expression in patients + 31-+ 60 d (7.24± 2.79) was significantly higher than that in patients + 14-+30 d (4.60+ 1.66) and +61-+ 100 d (3.86± 1.36) (P〈 0.05). Tim-3 mRNA expressions of grade 11-IV aGVHD group in patients + 14-+30 d, +31-+60 d, +61-+ 100 d were 9.54± 3.05, 10.14+3.28, 12.82±4.20, respectively, which in both + 14-+30 d and +61-+ 100 d were significantly higher than that of grade 0- I aGVHD and improved aGVHD groups (P〈0.05). In patients +31-+60 d, Tim-3 expression of grade II- IV aGVHD group was higher than improved aGVHD group (2.49±0.89), while no statistical difference when compared with grade 0- I aGVHD group (7.24±2.79). In grade I1 -IV aGVHD group, Tim-3 mRNA expression manifested no statistical difference among grades or organ involved (P〉0.05). ELISA results showed that plasma IFN-y and IL-2 concentrations were higher in gradeII - 1V aGVHD group than of other groups, while no significant difference existed between grade 0- I aGVHD and improved aGVHD groups (P〈0.05). Conclusion Tim-3 played an important role in the process of aGVHD.
