目的探讨妊娠期肝内胆汁淤积症(ICP)合并乙肝病毒(HBV)感染对孕妇及围生儿的影响。方法选择82例ICP合并HBV感染孕妇为观察组,55例单纯ICP孕妇为对照组,观察两组孕妇围产期并发症、围生儿情况及产妇产前、产后1周的丙氨酸氨基转移酶(ALT)、总胆红素(TBIL)、甘胆酸(CG)、天冬氨酸氨基转移酶(AST)以及直接胆红素(DBIL)等肝功能相关指标。结果 1观察组产妇并发妊娠高血压(24.4%)、剖宫产(92.7%)、早产(68.3%)以及发生产后出血的概率(28.0%)均显著高于对照组(P〈0.05)。两组低体重新生儿、Apgar评分、新生儿窒息比较,差异无统计学意义(P〉0.05)。2观察组产妇产前ALT、TBIL、TBA、AST以及DBIL等肝功能相关指标均显著高于对照组(P〈0.05)。产后1周,两组肝功能相关指标均显著降低,与产前比较,差异具有统计学意义(P〈0.05)。两组间比较,观察组各项肝功能相关指标均显著高于对照组(P〈0.05)。结论 ICP合并HBV感染孕妇围产期并发症发生率高,肝功能损伤重,除实施有效措施阻滞HBV母婴传播外,同时还应密切监测孕妇病情,采取积极的预防和治疗措施。
Objective To investigate the influence of intrahepatic cholestasis of pregnancy( ICP) combined with HBV infection on pregnant women and perinatal infants. Methods Eighty- two ICP pregnant women combined with HBV infection were chosen as the observation group,55 pregnant women with simple ICP were as control. Perinatal complications,perinatal infants status and ALT,TBIL,CG,AST,DBIL and other liver function parameters at 1 week prenatal and postpartum were compared. Results The rates of pregnancy- induced hypertension( 24. 4%),cesarean section( 92. 7%),premature birth rate( 68. 3%) and the probability of postpartum hemorrhage( 28%) in observation group were significantly higher than those in control group( all P〈0. 05). There were no significant difference in low birth weight,Apgar scores,neonatal asphyxia between two groups( P〉0. 05). Prenatal maternal ALT,TBIL,TBA,AST and DBIL in the observation group were significantly higher than those in control group( all P〈0. 05). One week postpartum,liver function parameters in two groups were significantly decreased; compared with prenatal values,the difference was statistically significant( all P〈0. 05). The liver function parameters of the observation group were significantly higher than those of control group( all P〈0. 05). Conclusion Pregnant women who has ICP combined with HBV infection had higher perinatal complications and heavier liver function injury,In addition to the implementation of effective measures to block mother to child transmission of HBV,the pregnant condition should be monitored closely.
