常规二维放疗和三维适形调强放疗对食管癌的疗效分析

目的探讨三维适形调强放疗(3D.IMRT)与二维常规放疗治疗食管癌的临床效果及安全性。方法86例食管癌患者随机分为3D.IMRT组和二维常规放疗组(普放组),比较2组近期疗效、远期疗效和毒副反应的差异。结果3D—IMRT组总有效率显著高于普放组;普放组放射性肺炎发生率显著高于3D-IMRT组;3D—IMRT组1、2、3、5年局部控制率及1、2、3年生存率显著高于普放组。结论与二维常规放疗比较,3D-IMRT能明显改善食管癌的近期疗效,提高局部控制率和生存率,并能有效减轻周围组织放疗损伤。

英文摘要:

Objective To explore the therapeutic effects and safety of three - dimensional intensity modulated radiation therapy (3D-IMRT) and two -dimensional general radiotherapy on esophageal carcinoma. Methods Eighty - six esophageal cancer patients were divided into two groups : 3 DIMRT group and two - dimensional conventional radiotherapy group ( general radiotherapy group). The differences between short- term and longterm therapeutic effects as well as toxic and side effects in two groups were compared. Results The total effectiveness rate of 3 D-IMRT group significantly was higher than that of general radiotherapy group. The incidence of radiation pneumonitis in general radiotherapy group was significantly higher than that in 3 D-IMRT group. The 1,2, 3,5 - year local control rates and 1,2, 3 - year survival rates in 3D-IMRT group were significantly higher than those in general radiotherapy group. Conclusion Compared with two -dimensional conventional radiotherapy, 3DIMRT can markedly enhance the short -term therapeutic effects of esophageal carcinoma, improve its local control rate and survival rate, and effectively alleviate the damage of surrounding tissues induced by radiotherapy.
