目的:应用统计学判别分析方法,筛选53项免疫学指标,建立再生障碍性贫血(aplastic anemia,AA)的中医证型判别方程,使AA中医证型客观量化。方法:采用流式细胞技术,对AA患者34例,健康对照人群34名外周血的免疫学指标进行检测,同时对AA患者进行辨证分型,以判别分析方法建立AA辨证分型的判别方程,对中医证型进行客观量化。结果:筛选出CD45RA^+绝对值、CD3^+CD25^+绝对值、CD3^+CD25^+(%)、CD3^+HLA—DR^+绝对值和CD4^+γδT(%)等5个最有意义免疫指标;建立AA中医证型的空间分布位置图;组成AA辨证分型的判别方程,对中医证候进行客观量化,其判别总正确率为76.5%。结论:应用判别分析方法建立了AA中医辨证各型判别方程,使AA辨证分型得以客观量化,具有临床实际意义。筛选出与辨证分型最相关的5个免疫学指标,与AA免疫学发病机制关系密切,说明AA中医辨证分型存在物质基础。
Objective: To build up discriminatory formulas for TCM differentiation type(TCMSDT) of aplastic anemia(AA), screen out immuno-indexes and quantify TCMDT of AA. Methods: Under the cytometry technique, immunoindexes in peripheral blood of 34 AA patients and 34 healthy persons were detected. Then build up discriminatory formulas to quantify TCMSDT of AA, analyse relations between these 2 subsets and TCMDT. Results: Firstly, absolute value of CD45RA^+, CD3^+CD25^+ and CD3^+HLA-DR^+, CD3^+CD25^+ ( % ) and CD4^+γδ T ( % ) are singled out to make up discriminatory formulas. Secondly, spatial distribution graph of TCMDT of AA is drawn. Thirdly, discriminatory formulas of TCMDT is made up, the total correct ratio is 76.5%.Conclusion: Discriminatory formulas of TCMDT were set up by using method of discriminatory analysis, 5 immuno-indexes which had remarkable correlation with TCM differentiation type were screened out, which had clinical significance obviously.
