目的:探讨社区成年人盐敏感性高血压的相关因素,特别是血浆神经肽Y(NPY)和脑钠肽(BNP)水平变化与盐敏感性高血压的关联。方法该研究于2014年12月至2015年10月共调查山西运城同德医院及延安大学咸阳医院管辖的社区居民1986名,发现原发性高血压患者100例,盐敏感性高血压患者65例(改良Sullivan 急性口服盐水负荷试验评定),同时从高血压患者家属中选取105名(无高血压)作为对照组。对受试者进行问卷调查,并测量身高、体重、腰围及血压,检测血清肌酐(Cr)水平、空腹血糖(FBG)、NPY以及BNP。比较3组一般情况,采用多因素Logistic回归分析盐敏感性高血压的影响因素。结果两组高血压患者的Cr、NPY以及BNP水平均高于正常对照组人群;盐敏感性高血压患者的Cr、NPY水平在3组研究对象中最高,高血压家族史的人数比例也最高,差异有统计学意义(P〈0.05);原发性高血压患者的BNP水平最高,差异有统计学意义(P〈0.05)。随着高血压级别升高,盐敏感性高血压患者的NPY水平和BNP水平均增高,差异有统计学意义(P〈0.05)。Logistic回归模型进行分析,单因素和多因素分析结果均提示,FBG低者发生盐敏感性高血压的概率低;而NPY、BNP水平增高、血清Cr水平异常以及高血压家族史都是盐敏感高血压发生的危险因素。结论 NPY、BNP水平增高等因素均可增加盐敏感性高血压发生的概率。
