2006-2008年安庆地区散发性戊型肝炎病毒血症检测与序列分析

目的了解安庆地区散发性戊型肝炎病毒血症的检出率与基因型的分布。方法收集2006-2008年间安庆市立医院血清戊肝IgM抗体阳性患者的血清(包括单份血清和系列血清)共145份,采用巢式RT-PCR方法检测HEVRNA,并扩增494-nt核苷酸片段,进行基因分型和同源性比对。结果在57名血清戊肝IgM抗体阳性的患者血清中,HEVRNA总检出率为47.37%;男性与女性分别为50.00%、40.00%,两者间无显著性差别(χ^2=0.44,P=0.51)。其中,单份/首份血清的检出率最高(26/57,45.61%),第2份血清较低(8/35,22.86%),第3份及以后的血清几乎未检出阳性(1/53,1.89%)。所有35条HEV核苷酸序列均为基因Ⅳ型,与2007-2008年间获得的27株安庆地区猪基因Ⅳ型HEV共同比对后发现,人序列之间、猪序列之间、人与猪序列之间的同源性都约为86%;不同年份、不同宿主(人与猪)的HEV核苷酸序列间的同源性均在84%~88%之间。结论基因Ⅳ型HEV是安庆地区散发型戊肝的主要病毒株。当地人与猪HEV序列的同源性较高,提示HEV在人与猪之间存在相互传播的可能。

英文摘要:

To determine the viremia of soradic heatitis E virus in atients' serum and genotye of the virus isolated in Anqing,Anhui rovince,a total of 145 atients' serum samles(including single samles and serial samles from 57 atients) in which each first samle was ositive for HEV-IgM antibody were collected in Anqing Municial Hosital between 2006 and 2008 for further study.HEV RNA was detected by nested RT-CR and a 494-nt fragment was amlified for further genotying and hylogenetic analysis.HEV RNA was detected from 27 out of 57(47.37%) atients' serum samles.The ositive rates were 50.00% and 40.00% in male and female atients resectively with no significant difference(χ^2=0.44,=0.51).The ositive rate in the first serum samles was higher(26/57,45.61%) than that in the second(8/35,22.86%),third and the other(1/53,1.89%) ones.All the 35 sequences were classified into genotye-IV HEV.hylogenetic analysis revealed that those human strains shared 86% nucleotide similarity with 27 swine HEV strains which were isolated locally between 2007 and 2008.The interior similarities among the human strains and swine strains were 86%.The HEV strains isolated from different hosts between 2006 and 2008 all shared 84%-88% similarities on average.It's suggested that genotye-IV HEV is the rincile genotye for the soradic heatitis E virus infection in Anqing.The great similarities of HEV strains among local human and swine imly bilateral transmission of HEV between human and swine.
