为了利用瞬态诱发耳声发射(TEOAE)信号对耳蜗性和蜗后性两种感音神经性耳聋进行病变诊断和定位,提出了一种基于AR谱分析的方法,用于对感音神经性耳聋病变进行诊断和定位.首先,获取听力正常耳信号的对数AR谱-频率曲线的正常值范围,并获得在对侧刺激声作用下,信号受抑制的对数AR谱-频率曲线的正常值范围;然后,计算某一受试耳在有无对侧刺激声作用下的TEOAE信号的对数AR谱-频率曲线及受抑制的对数AR谱-频率曲线;最后,检测受试耳的两条曲线是否分别在正常值范围内,由此对受试耳的耳蜗及内侧橄榄耳蜗系统功能进行诊断定位.此检测结果与目前临床上综合利用耳声发射、脑干听觉诱发响应(ABR)和纯音测听的诊断结果有很好的一致性.
In order to diagnose and locate two main defective sensorineural hearing loss sites,cochlear hearing loss and retrocochlear hearing loss,a technique that is based on AR spectrum analysis for the signals of Transient Evoked Otoacoustic Emissions(TEOAE) and TEOAE suppressed by contralateral acoustic stimulation(CAS) was proposed.The authors first obtained the range of normal values of logarithmic AR spectrum frequency curve of normal hearing,as well as the range of normal values of AR spectrum frequency curve of the signals suppressed by CAS;then calculated the logarithmic AR spectrum frequency curve of TEOAE signals and that of the suppressed logarithmic AR spectrum frequency curve of a testee's hearing,with or without CAS;and then measured if either of the two curves is within the range of normal values,thereby the testee's cochlea and internal olivary cochlea system were diagnosed and positioned.The test results were comparatively consistent with the clinical diagnosis result got by integrating pure hearing test with OAE and Auditory Brainstem evoked Response(ABR).
