目的:探讨氟-18标记的脱氧葡萄糖(18F—FDG)放射性核素行正电子发射断层代谢显像(PET/CT)对冠心病诊断及治疗决策的作用。方法:对11例冠心病患者进行完整的病史采集及体格检查,行18F-FDGPET/CT和超声心动图检查,通过半定量评分的方法确定心室各节段葡萄糖代谢和心肌运动情况。并在1周内行冠状动脉造影术,结合心肌代谢显像结果制定治疗方案。结果:11例患者均获得较高质量的18F—FDGPET/CT心肌代谢显像结果,共46心肌节段出现代谢异常,其中13个心肌节段为代谢缺损,PET/CT与心脏彩超检查在异常心肌检出率方面有较好的一致性(P=0.60)。通过半定量评分比较,1分级别18F—FDGPET检出率为8.0%,高于心脏彩超的4.8%;对于评分正常、2级和3级的检出率两者较为接近。在各个异常代谢节段中,心尖存活指数最高,为(1.55±1.21)。结论:18F—FDGPET/CT可以准确评估冠心病患者心肌存活状况,结合常规检查能进一步对疑难冠心病进行诊断。
Objective:To evaluate the application value of fluorine-18 fluorodeoxyglucose(18F-FDG) positron emission tomography (PET) in the diagnosis and treatment of coronary heart disease (CHD). Methods: 11 CHD patients underwent 18F-FDG PET/CT and ultrasonic cardiogram examinations were retrospectively analyzed. Myocardial glucose metabolism and the left ventricular wall motion were confirmed by semiquantitative classification. The therapeutic regimen was carried out according to coronary angiography and 18F_FDG PET/CT. Results. All patients received high quality of 18F_FDG PET/CT myocardial metabolic imaging. 46 myocardial segments exhibited abnormal metabolism in PET imaging, including 13 cases of metabolic defect. Abnormal myocardium detected by PET/CT was in consistence with echocardiography ( P =0.60). The detection rate of 18F-FDG PET (8.0%) in score 1 group was higher than that of echocardiography (4. 8%). The survival index of apex cordis was the highest (1.55±1.21) in abnormal myocar-dia. Conclusion: 18F-FDG PET imaging could accurately assess myocardial viability in CHD patients, and diagnose knotty CHD when combined with routine examination.
