目的 探讨剪切波超声弹性成像在深静脉血栓分期中的价值.方法 95例患者共120条深静脉血栓患肢,根据临床症状起始时间分为急性组、亚急性组和慢性组,剪切波超声弹性成像测量血栓杨氏模量值,对比分析,并利用ROC曲线分析不同血栓分期间的杨氏模量值诊断截点.结果 急性期、亚急性期、慢性期血栓杨氏模量平均值分别为(7.99±2.62) kPa、(13.52±2.31)kPa、(24.61±6.93) kPa,三者之间差异有统计学意义(P<0.05),血栓形成时间越长,杨氏模量值越大.以10.35 kPa作为诊断急性血栓的杨氏模量值截点时,敏感性86.67%,特异性97.44%;以17.05 kPa为诊断慢性血栓的杨氏模量值截点时,敏感性94.44%,特异性92.31%.结论 杨氏模量值随血栓形成时间增加而增加,急性与亚急性血栓之间、亚急性与慢性血栓之间存在敏感性和特异性均较高的杨氏模量值诊断截点.

英文摘要:

Objective To discuss the value of ultrasound shear wave elastography imaging in the staging of deep vein thrombosis.Methods 120 extremities of 95 patients were enrolled in the present study.Based on the duration from occurrence of clinecal symptoms,the thromboses were classified as acute,subacute and chronic.The elastic moduli were all measured and the diagnostic performance was assessed using ROC curves to get the cut-off values.Results The mean value of elastic moduli in acute,subacute and chronic thrombosis was(7.99±2.62)kPa,(13.52±2.31)kPa,and(24.61±6.93)kPa respectivel,with a significant difference among them(P〈0.05).The longer the thrombus existed,the greater the elastic modulus was.The cut off value was 10.35kPa for acute thrombosis with a sensetivity of 86.67 and a specificity of 97.44 and 17.05 kPa for chronec thrombosis with a sensitivity of 94.44 and a specificity of 92.31.Concousions The modulus of elasticity increases with the duration of thrombosis.There are cut-off values between acute thrombosis and subacute thrombosis and between and between subacute thrombosis and chronic thrombosis,of which the sensitivity and apecificity are both high.
