This paper researched some typical variations and corresponding measures in the course of osteosarcoma preoperative chemotherapy. The workflow model based on extended （event-condition-action, ECA）rules was constructed and some descriptions of the osteosarcoma preoperative chemotherapy were given. Moreover, a workflow engine based on rule was designed, its structures and working process were explained, and the core modules were introduced in details. Finally, a case study was used to validate the adaptive mechanisms of this workflow engine.
