目的探讨七氟醚预处理对大鼠部分肝缺血/再灌注(ischemia/reperfusion,I/R)肺损伤时肺组织基质金属蛋白酶9(matrix metalloprofeinases9,MMP-9)mRNA表达的影响。方法健康Wistar大鼠54只,体重270g-330g,采用随机数字表法分为3组(每组18只):假手术组(S组),仅游离肝门,但不阻断;肝I/R组(I/R组),采用阻断肝左、中叶(70%)血供30min后再灌注的方法建立大鼠部分肝I/R模型;七氟醚预处理组(Sev组),吸入2%七氟醚30min,停止吸入后制备部分肝YR模型。分别于再灌注30、60min和120min时随机处死6只大鼠,取肺组织,苏木精伊红(hematoxylin-eosin,HE)染色,光镜下观察肺组织病理学结果,进行肺损伤评分,测肺组织湿/干重比(wet/dry,W/D)、肺组织髓过氧化物酶(myeloperoxidase,MPO),并以逆转录聚合酶链式反应(reverse transcription-polymemse chain reaction,RT-PCR)方法检测肺组织MMP-9mRNA的相对表达量。结果与S组比较,I/R组再灌注30、60、120min时点均发生肺损伤,且MPO含量[分别为(1.65±0.69)、(1.79±0.24)、(2.54±0.65)U/g]、MMP-9mRNA表达量[分别为(1.79±0.52)、(3.60±0.45)、(3.96±1.62)]均增高(P〈0.05);与I/R组比较,Sev组再灌注各时间点肺损伤减轻,MPO含量[分别为(1.27±0.33)、(1.49±0.13)、(2.04±0.14)U/g]、MMP-9mRNA表达量[分别为(1.74±0.76)、(2.18±1.72)、(3.49±0.31)]均降低(P〈0.05)。结论七氟醚预处理可减轻部分肝I/R中的肺损伤,抑制肺组织MMP-9mRNA的表达可能是其机制之一。
Objective To investigate the effects of sevoflurane preconditioning on pulmonary matrix metalloproteinases 9 (MMP-9) mRNA during partial hepatic isehemia/reperfusion (I/R) lung injury in rats. Methods Fifty-four male wistar rats weighing 270 g-330 g were randomly divided into 3 groups (n=18): sham operation group (group S, the hilum of liver was isolated but not clamped), partial hepatic I/R group (group I/R, the left and median lobe of liver was occluded for 30 rain followed by reperfusion), sevoflurane preconditioning group (group Sev, 2%sevoflurane was inhaled for 30 rain, and partial heptic I/R was induced after the end of inhalation in group Sev). Six rats in each group were chosen at 30, 60 rain and 120 min of reperfusion and sacrificed, and lungs were removed for microscope examination of pathologic changes which were scored and for determination of wet/ dry lung weight (W/D) ratio myeloperoxidase (MPO) activity (by colorimetrie method) and the expression of MMP-9 mRNA[by reverse transcription-polymerase chain reaction (RT-PCR)1. Results Compared with group S, the lung injury appearred in the group I/R at 30, 60 min and 120 rain, and MPO activity [ (1.65±0.69), (1.79±0.24), (2.54±0.65) U/g] and expression of MMP-9 mRNA [ (1.79±0.52), (3.60±0.45), (3.96±1.62) ] increased significantly (P〈0.05). Compared with group I/R, the degree of lung tissue injury relieved in group Sev at 30, 60 rain and 120 min, MPO activity [ (1.27±0.33), (1.49±0.13), (2.04±0.14) U/g] and expression of MMP-9 mRNA [(1.74±0.76), (2.18±1.72), (3.49±0.31)1 decreased significantly (P〈0.05). Conclusions Severe acute lung injury after partial hepatic I/R in rats is attenuated by sevoflurane.An inhibition of MMP-9 mRNA expression in lung tissues may be one of the mechanisms for that.
