目的评价正念训练对于更年期妇女的身心健康指标的影响效果,探讨建立以正念训练为主的更年期妇女的心理保健模式。方法以社区中招募的更年期妇女为研究对象,随机分为两组,实验组参加8周的正念训练,同时设置对照组,比较两组的更年期症状、心理状况、血压等指标的变化差异。结果①更年期症状改善状况:实验组和对照组Kupperman评分总分干预后均较干预前显著降低(t值分别为-3.082、-2.625,均P〈0.05),实验组干预后的情绪波动、骨关节痛、头痛和心悸评分均较干预前显著降低(t值分别为-2.244、-2.301、-4.382,均P〈0.05),而对照组上述评分无显著性差异(t值分别为-1.000、-0.563、-1.385,均P〉0.05);②心理改善状况:实验组干预后的抑郁自评量表总分、特质-焦虑量表评分均显著下降(t值分别为-3.487、-3.131,均P〈0.05),总体幸福感量表总分显著上升(t=2.922,P〈0.05),而对照组抑郁自评量表总分、总体幸福感量表总分和特质-焦虑量表评分无显著性差异(t值分别为-0.952、0.450、-0.244,均P〉0.05);③医学检查指标变化状况:实验组的舒张压干预后较干预前显著降低(t=-2.561,P〈0.05),而对照组舒张压比较无统计学差异(t=-1.191,P〉0.05)。结论正念训练可以降低改善更年期症状评分,对于感觉异常、情绪波动、心悸、皮肤蚁走感等更年期症状有改善的趋势,对于提高更年期妇女的幸福感、降低焦虑、抑郁情绪有改善趋势。
Objective To evaluate the effect of mindfulness training on physical and mental health indicators of menopausal women and to discuss the building of mindfulness training-based mental health mode for this group. Methods Women recruited from communities were randomly divided into control group and experimental group, and the later took 8-week mindfulness training course. Menopausal symptoms, mental state and blood pressure were compared between two groups. Results After intervention the Kupperman score decreased significantly in both groups compared with that before intervention ( t value was - 3. 082 and - 2. 625, respectively, both P 〈 0. 05 ). In the experimental group mood swing, joint pain, headache and cardiopalmus scores were significantly decreased after intervention (t value was -2. 244, -2.301and -4. 382, respectively, all P 〈 0.05 ), but there was no significant difference in the control group (t value was - 1. 000, - 0.563 and - 1. 385, respectively, all P 〉 0.05). The scores in Self-rating Depression Scale and State-Trait Anxiety Inventory were significantly reduced in the experimental group ( t value was - 3. 487 and - 3. 131, respectively, both P 〈 0.05 ) but the score of Index of Well - being was significantly increased after intervention ( t = 2. 922, P 〈 0.05 ). However, there was no significant difference in the control group (t value was - 0. 952, 0.450 and - 0.244, respectively, all P 〉 0.05 ). After intervention, the systolic blood pressure decreased significantly in the experimental group ( t = - 2.561, P 〈 0.05 ), but the difference was not significant in the control group ( t = - 1. 191, P 〉 0.05 ). Conclusion Mindfulness training can reduce the score of menopausal symptoms and improve paresthesia, mood swing, cardiopalmus and formication. It is an effective psychological intervention in terms of improving the happiness and reducing anxiety and depression of menopausal women.
