2016年云南畹町口岸地区登革热疫情的病原学和流行病学调查

目的阐明2016年云南省畹町口岸地区登革热流行特征和登革病毒血清型、基因型及传播来源。方法收集登革热病例资料,采集患者急性期血清样本,用RT-PCR法检测登革病毒核酸,并进行登革病毒C/Pr M基因核苷酸序列测定和进化分析。用布雷指数法监测伊蚊幼虫密度。结果 2016年畹町地区共确诊登革热病例36例,其中缅甸输入性病例34例(94.44%),本地感染病例2例(5.56%)。流行时间为5-12月,主要为10和11月(27例)。布雷指数5-9月为3.33~8.55,其他月≤2.25。从患者血清中获得11株登革病毒的C/Pr M基因核苷酸序列,进化分析表明,9株为登革病毒血清型1型(DENV-1)中的基因Ⅰ型(Genotype-Ⅰ,G-Ⅰ),1株为登革病毒血清型2型(DENV-2)的美洲基因型(American genotype,AMG),1株为登革病毒血清型4型(DENV-4)的G-Ⅰ。DENV-1和DENV-4均与近几年云南省瑞丽市、临沧市和缅甸同型流行株具有较近的亲缘关系,DENV-2与墨西哥和柬埔寨流行株亲缘性较近。结论 2016年畹町口岸地区存在DENV-1 G-Ⅰ、DENV-2 AMG和DENV-4 G-Ⅰ的流行,应加强中缅边境地区登革热跨境传播的防控。

英文摘要:

Objective To understand the epidemiologic features of an epidemic of dengue fever(DF) in Wanding port area of Yunnan province,China,2016 with dengue virus(DENV) serotype,genotype and transmission source.Methods The data of DF cases in Wanding were collected. The serum samples from acute stage of these patients were collected to detect the viral nucleic acid by RT-PCR assay. The phylogenetic tree was analyzed and generated based on the nucleotide sequence of C/Pr M gene fragments of DENV. The density of Aedes larvae was monitored with the method of Breteau index(BI). Results A total of 36 DF cases were reported in Wanding in 2016. Of them,34 were imported cases(94.44%) from Myanmar and 2 were indigenous cases(5.56%). Epidemic month of DF was from May to December,and the main month was October and November. The BI was 3.33-8.55 from May to September,and no more than 2.25 in other months. The nucleotide sequences of C/Pr M of 11 strains were obtained from DF cases. Phylogenetic analysis indicated that 9 virus strains belonged to genotype Ⅰ(G-Ⅰ) of DENV serotype 1(DENV-1),and 1 virus strains belonged to American genotype(AMG) of DENV serotype 2(DENV-2),and 1 virus strains belonged to G-Ⅰ of DENV serotype 4(DENV-4). The DENV-1 and DENV-4 strains from Wanding had a closer genetic relationship with the DENV-1 and DENV-4 strains from Ruili city,Lincang prefecture of Yunnan province and Myanmar in recent years. The DENV-2 strain obtained from Wanding had a closer genetic relationship with the DENV-2 strains from Mexico and Cambodia. Conclusion It is confirmed epidemic of DENV-1 G-Ⅰ,DENV-2 AMG and DENV-4 G-Ⅰ in Wanding port area of Yunnan province,China in 2016. Ⅰt is should strengthen control the cross-border spread of DF in this region.
