目的研究产碳青霉烯酶鲍氏不动杆菌的耐药性与耐药表型的关系。方法采用改良Hodge试验和乙二胺四乙酸(EDTA)协同增强试验法,筛选产碳青霉烯酶鲍氏不动杆菌并进行表型研究。结果 130例鲍氏不动杆菌株中,对碳青霉烯类耐药率40.0%,其中高水平耐药类30.0%;喹诺酮类60.0%;四环素66.9%;大环内酯类62.3%;β-内酰胺类70.0%;改良Hodge试验阳性率69.2%,EDTA协同增强试验阳性率98.0%。结论鲍氏不动杆菌产金属酶,是对亚胺培南和美罗培南等碳青霉烯类抗菌药物耐药的主要原因之一,但表型筛选不能对耐药机制做出完全确定,必须通过分子生物学检验技术,才能最终确定耐药的根本机制。
OBJECTIVE To study the relationship between drug resistance of carbapenemases-producing Acinetobacter baumannii(ABA) and drug resistant phenotypes.METHODS Carbapenemases-producing ABA was screened by the Modified Hodge Test and EDTA-disk synergy and reinforcement test.RESULTS The resistance rate of the carbapenems antibiotics was 40.0% and 30.0% was high resistance.The resistance rate to quinolinones was 60.0%;tetracycline was 66.9%;macrolide was 62.3%;Beta-Lactam was 70.0%.The positive rate of the Modified Hodge Test was 69.2% and the positive rate of the EDTA-disk synergy and reinforcement test was 98.0%.CONCLUSION The production of carbapenemases is among the main mechanisms of carbapenemases resistance in ABA,but it must be determined definitely by molecular biology.
