[目的]根据蛋清实验探讨不同功率时间组合射频等离子针刺最佳消融量效参数,评价此组合射频等离子针刺治疗非特异性腰痛的疗效。[方法]根据射频等离子针刺蛋清实验,测量不同功率射频等离子针刺使蛋清产生变性所需时间及产生变性的最大长度、宽度,以确定最佳消融量效参数并为临床应用提供指导。同时,回顾性分析2014年3月~2015年3月应用此参数组合射频等离子针刺治疗的非特异性腰痛患者58例,观察指标包括VAS评分、腰椎活动度及红外热成像温度,应用SPSS 19.0软件对治疗前后各疗效指标进行统计分析。[结果]蛋清实验:射频功率3 W以上时才有治疗意义,功率4~6 W射频需发射2 s以上才起作用,功率7~9 W需发射1 s以上起作用,功率10 W发射低于1 s即起作用,不同功率时间组合产生蛋清热凝物的最大长度、宽度存在差异。临床研究:治疗后VAS评分较治疗前减小,差异有统计学意义(P〈0.01);腰椎活动度较治疗前增加,差异具有统计学意义(P〈0.01);腰部平均温度较治疗前降低,差异有统计学意义(P〈0.01)。[结论]射频等离子针刺随功率升高治疗时间缩短,但治疗范围呈缩小趋势,功率4~7 W、作用时间2 s以上时,射频等离子针刺消融量效关系最佳;此功率时间组合射频等离子针刺治疗非特异性腰痛并发症少、临床疗效确切。
[Objective] To explore the best intensity - time parameter of plasma radiofrequency in egg white test, then to investigate the clinical outcomes of f plasma radiofrequency acupuncture using selected parameters in therapy of non-specif- ic low back pain. [Methods] An egg white test was conducted to measure the denature change in length and width using dif- ferent intensity-time combination of plasma radiofrequency for exploring the suitable parameters of radiofrequency in clinical practice. Afterwards, a total of 58 patients diagnosed as non-specific low back pain were treated by plasma radiofrequency acu- puncture in the selected intensity-time combinations from March 2014 to March 2015. The VAS score, range of lumbar spinal motion and temperature variation in the infrared thermal imaging were measured and analyzed by SPSS 19.0 before and after treatment. [Results] The egg white test indicated that effective changes occurred only if the radiofrequency intensity was larg- er than 3W. As the radiofrequency intensity was set as 4W-6W, 7w-gw and 10W, it worked as time elapsed more than 2 s, more than ls, less than ls respectively. Different intensity - time combinations produced significant differences in thermoset change of egg white in the length and width. In clinical setting, the VAS score decreased significantly after treatment (P〈 0.01), whereas the range of lumbar spinal motion increased significantly after treatment (P〈0.01) . Additionally, the average temperature in waist decreased significantly after treatment (P〈0.01) . [Conclusion] As the radiofrequency intensity is set as 4W-7W and the treatment time last for more than 2s, the optimal treatment outcome would be achieved for non- specific low back pain.
