目的探讨PureceptionTM密度梯度离心法优选精子在夫精人工授精(AIH)中的应用价值。方法采用Pureception^TM密度梯度离心法优选精子,应用于672对不孕夫妇的1 360个AIH周期治疗。结果经Pureception^TM密度梯度离心法优选精子后,精子浓度由(54.86±26.03)×10^6/mL提高到(63.89±34.40)×10^6/mL、前向运动(PR)精子百分比由(32.47±9.31)%提高到(69.78±5.10)%,其差异均具统计学意义(P〈0.05)。PR精子回收率为(34.08±3.95)%。672对不孕夫妇共进行1 360个AIH周期,周期临床妊娠率16.62%(226/1 360)。优选精子前根据精子浓度分为3组:≥15×10^6/mL组(Ⅰ组)、(≥10,〈15)×10^6/mL组(Ⅱ组)、〈10×10^6/mL组(Ⅲ组),临床妊娠率分别为17.13%、16.05%、4.26%,Ⅲ组明显低于其他2组(P〈0.05);根据PR精子百分比分为3组:≥32%组(A组)、(≥10,〈32)%组(B组)、〈10%组(C组),临床妊娠率分别为17.53%、16.55%、4.65%,C组明显低于其他2组(P〈0.05)。优选精子后根据PR精子总数分为两组:≥10×10^6组、〈10×10^6组,临床妊娠率分别为17.33%、10.22%,两组之间差异具有统计学意义(P〈0.05)。结论 Pureception^TM密度梯度离心法是一种较好的精子优选方法,应用于AIH可获得较为满意的临床妊娠率。优选精子前精子浓度≥10×10^6/mL、PR精子≥10%,及优选精子后PR精子总数≥10×10^6,可获得较理想的临床妊娠率。
Objective To probe into the application of PureceptionTM gradient centrifugation technique for sperm separation in artificial insemination by husband(AIH).Methods Prepare the semen by PureceptionTM gradient centrifugation technique,and used for 1360 AIH cycles in 672 infertile couples.Results After preparing the semen by PureceptionTM gradient centrifugation technique,the sperm concentration was increased from(54.86±26.03)×10^6/mL to(63.89±34.40)×10^6/mL,the rate of PR sperm was increased from(32.47±9.31)% to(69.78±5.10),the differences were both statistically significant(P〈0.05).the recovery rate of PR sperm was(34.08±3.95)%.672 infertile couples received 1360 AIH cycles,and the clinical pregnance rate was 16.62%(226/1360).According to the sperm concentration before preparing the semen,the cycles were divided into 3groups:≥15×10^6/mL(GroupⅠ),(≥10,15)×10^6/mL(GroupⅡ)and 10×10^6/mL(Group Ⅲ),the clinical pregnancy rates were17.13%,16.05% and 4.26%,respectively,Group Ⅲ was significantly lower than the other two groups(P〈0.05);According to the rate of PR sperm,the cycles were divided into 3groups:≥32%(Group A),(≥10,32)%(Group B)and10%(Group C),the clinical pregnancy rates were 17.53%,16.55% and 4.65%,respectively,Group C was significantly lower than the other two groups(P〈0.05).According to the number of PR sperm after preparing the semen,the cycles were divided into 2groups:≥10×10^6 and 10×10^6,the clinical pregnancy rates were 17.33% and 10.22%,the difference was statistically significant(P〈0.05).Conclusion PureceptionTM gradient centrifugation technique is a better way for sperm separation,and the clinical pregnance rate is satisfactory when it is used in AIH.When the sperm concentration is≥10×10^6/mL and the rate of PR sperm is≥10% before preparing the semen,and the number of PR sperm is ≥10×10^6 after preparing the semen,patients will get a more ideal pregnancy rate.
