目的 探讨适宜临床应用的人工耳蜗植入者噪声环境下普通话言语测听( mandari n heari ng i n noise t est , MHI NT ) 方案。方法 对 10 名母语为普通话的语后聋人工耳蜗植入者进行 MHI NT 测试。测试方案 1 : 由易到难的顺序进行安静环境言语识别率测试( s peech r ecogniti on scor es , SRS )→ 固定信噪比 SRS 测试 → 言语识别阈测试( r ecepti on t hr es hol d of s ent ences , RTS ) ; 测试方案 2 : 由难到易的顺序进行 RTS 测试 → 固定信噪比 SRS 测试 → 安静环境 SRS 测试。优化测试方案: 预测试→ 对象以言语识别率得分高低分级 →正试测试。结果① 测试方案 1 : 全体受试者安静环境 SRS 测试的完成率 100 % ( 10 / 10 ) , 固定信噪比 SRS 测试的完成率 50 % ( 5 / 10 ) ; 4 名可完成固定信噪比 SRS 测试的受试者接受 RTS 测试, 2 名完成测试( 50 % , 2 / 4 ) ;② 测试方案 2 : 全体受试者依次接受 RTS 测试( 完成率 20 % , 2 / 10 ) 、噪声环境 SRS 测试( 完成率 50 % , 5 / 10 ) 和安静环境 SRS 测试( 完成率为 100 % , 10 / 10 ) ; ③优化测试方案: 10 名对象根据预测试 SRS 得分分级后, 5 名受试者接受并完成安静环境 SRS 测试( 完成率 100 % , 5 / 5 ) ; 5 名受试者接受并完成噪声环境 SRS 测试( 完成率 100 % , 5 / 5 ) , 其中 4 名进一步接受自适应 RTS 测试, 2 名完成测试( 完成率 50 % , 2 / 4 ) 。结论 根据预测试结果对受试者的言语能力进行分级, 选取难度适中的测试方案进行测试, 可提高言语测试的效率。根据助听装置使用者的言语能力为其制定个性化的测试参数和合理的评估流程, 将有利于 MHI NT 测试在临床言语识别评估工作的推广使用。
Objective To investigate the optimized testing procedure of MHINT(Mandarin hearing in noise test) in order to evaluate the speech understanding ability of cochlear implant reciepients in clinic audiology assessment.Methods Two test procedures of MHINT were designed to the assessment of 10 post-lingual deafness CI recipients.The "Procedure 1" following the "easy to difficult" order includes SRS(speech recognition scores) test in quiet condition,SRS test in fixed SNR(signal noise ration),RTS(reception threshold of sentences) test in adaptive condition.Whereas "Procedure 2" following the "difficult to easy" order includes RTS test in adaptive condition,SRS test in fixed SNR,SRS test in quiet condition.Optimization testing procedure includes: prior test → classified the difficult level of speech test → hot test.Results ①Procedure 1:All subjects were tested and finished the SRS in quiet test(completion rate:100%),all participants were tested and half of them could finish the SRS in noise test (completion rate:50%),4 subjects were tested and half of them could finish the RTS(completion rate:50%).②Procedure 2:All subjects were tested for the RTS test(2 finished,completion rate:20%),SRS in noise test(5 subjcets finished,completion rate:50%) and SRS in quiet test(all subjects could finish,completion rate:100%),respectively.③Optimized testing procedure: All of the to participants were submitted to the prior test in order to classify the difficult level of speech test to be accepted.Five subjcets finished the SRS in quiet test(completion rate:100%),other 5 subjects accepted SRS in noise test and 4 of them could finished.These 4 subjcets further reciened the RTS test and 2 of them finished(completion rate:20%).Conclusion The personalized parameters and optimized testing procedure designed for CI recipients could improve the compleior rate of M-HINT test.Therefore audiologists will get more useful information for CI recipients'
