目的评价利普液基细胞学(LPT)技术联合肿瘤标志物检测在甲状腺细针穿刺(FNA)中的临床应用价值。方法 20例甲状腺FNA标本同时采用传统涂片和LPT技术制备细胞涂片。其中,14例用免疫细胞化学法测定穿刺标本中半乳糖凝集素3(Gal-3)与甲状腺过氧化物酶(TPO)的表达情况。结果与传统细胞涂片相比,LPT制片获得细胞数量较多,细胞呈单层均匀分布,结构清晰,涂片背景干净。Gal-3在高度怀疑恶性病变的甲状腺滤泡细胞中呈阳性表达,但在良性病变的甲状腺滤泡细胞中不表达或低表达。TPO在高度怀疑恶性病变的甲状腺滤泡细胞中不表达或低表达,但在良性病变的甲状腺滤泡细胞中呈中到强阳性表达。结论 LPT制片技术明显提高甲状腺FNA标本制片质量;LPT技术结合肿瘤标志物检测,可用于筛查和诊断甲状腺癌和甲状腺癌前病变。
Objective To investigate the value of application of detecting thyroid specific tumor markers and Liqui-prep test in thyroid fine needle aspiration biopsy. Methods Thyroid cell samples of 20 patients obtained by thyroid fine-needle aspiration method were examined with Liqui-prep test and conventional smear method at same time, of which 14 samples were examined with immunocytochemistry staining for detecting the expressions of Galectin-3 and TPO. Results Compared to conventional smear method, more cells were obtained by Liqui-prep test With well-distributed cell morphology and clear background and nucleus structure. Galectin-3 was strongly expressed in most thyroid follicular cells highly suspected to be malignant lesions, which was not or weak expressed in benign lesions. TPO was unexpressed in most malignant lesions but strongly expressed in benign lesions. Conclusion Liqui-prep test can significantly improve the smears quality of thyroid fine-needle aspiration. It would contribute to improve the diagnosis efficiency in fine needle aspiration of thyroid nodule when Liqui-prep test combined with immunocvtochemistrv is used.
