目的:帕金森病相关脑功能网络(PDRP)是反映“皮质-纹状体-苍白球-丘脑-皮质”环路和相关解剖/功能通路葡萄糖代谢变化的影像学标志物。为推广其临床应用,需验证不同PET重建算法对PDRP表达值计算的影响。方法:将58例帕金森病(PD)患者及22例正常对照的18F—FDGPET显像图像分别进行滤波反投影法(FBP)及迭代法重建,基于华山医院PET中心建立的PDRP,应用SSM/PCA软件计算受检者经不同重建方法得到的PDRP表达值。结果:FBP法与迭代法PDRP表达值的差异无显著的统计学意义(PD组P〉0.05;对照组(P〉0.05),两者呈显著正相关。PD组PDRP表达值显著高于对照组(FBP法:P〈0.05;迭代法:P〈0.05)。PDRP表达值与PD患者的UPDRS中的运动功能评分均呈显著正相关。结论:不同重建算法所得的PDRP表达值无显著统计学差异,不影响其在PD诊断及病情评估等中的临床效能,为PDRP在国内多中心的研究奠定了基础.
Purpose: Parkinson' s disease-related pattern (PDRP) is a newly discovered imaging biomarker, which can reflect the glucose metabolic change of "Cortico-striato-pallidal-thalamo-cortical" loop and related anatomical/functional pathway. The influence of different PET reconstruction algorithms on PDRP expression should be verified before it can be widely used in the clinic. Methods: The 18F-FDG PET images of 58 Parkinson' s disease (PD) patients and 22 heath control subjects (HC) were reconstructed by filtered back projection algorithm (FBP) and iterative algorithm (Iteration), separately. Based on the PDRP from PET center, Huashan Hospital, the PDRP expressions of different algorithms were calculated by SSM/PCA software. Results: There was no difference of PDRP expression between FBP and iteration (PD: P〉0.05; HC: P〉0.05). They had significantly positive correlation with each other both in PD and HC (PD: P〈0.05; HC: P〈0.05). The PDRP expression of PD group was significantly higher than that of HC group (FBP:P〈0.05; Iteration: P〈0.05). In addition, the PDRP expression had significantly positive correlation with PD patients' UPDRS motor function value (FBP: P〈0.05; Iteration: P〈0.05). Conclusion: The PDRP expression from different reconstruction algorithms had no significant difference. Additionally, the reconstruction algorithm will not affect the application of PDRP in diagnosis or assessment of PD, which is important for domestic multi-center researches.
