目的探讨ARK5和基质金属蛋白酶2(MMP-2)在小细胞肺癌(SCLC)组织中的表达及其临床意义。方法采用免疫组化方法检测106例SCLC癌组织(SCLC组)及40例正常肺组织(对照组)中ARK5和MMP-2的表达,分析ARK5、MMP-2的表达与患者病情分期及预后的关系。结果SCLC组ARK5和MMP-2阳性率分别为67.9%(72/106)和65.1%(69/106),对照组中分别为5.0%(2/40)和7.5%(3/40),差异均有统计学意义(χ2值分别为46.004和38.540,P〈0.05)。ARK5和MMP~2的阳性率与肿瘤直径、分化程度、浸润深度、淋巴结转移及肿瘤分期有关(P〈0.05)。Spearman秩相关分析显示,SCLC癌组织中ARK5与MMP-2呈正相关(r=0.878.P〈0.001)。Kaplan-Meier生存曲线显示,ARK5阳性组5年生存率为12.9%,阴性组为59.6%,差异有统计学意义(χ2=30.077,P〈0.001);MMP-2阳性组5年生存率为16.3%,阴性组为62.3%,差异有统计学意义(χ2=24.405,P〈0.001)。Cox多因素分析显示,ARK5、分化程度、肿瘤分期对SCLC患者顸后的影响有统计学意义(P〈0.05)。结论ARK5和MMP-2的表达与SCLC的分化程度、浸润深度、淋巴结转移和肿瘤分期密切相关,它们都对SCLC患者的生存率产生重大影响,同时ARK5是SCLC患者预后的独立危险因素。
Objective To investigate the expression of ARK5 and MMP- 2 in small cell lung cancer (SCLC) and its clinical significance. Methods Immunohistochemistry method was used to detect the expressions of ARK5 and MMP - 2 in 106 cases of SCLC tissues and 40 cases of normal lung tissues. The expressions of ARK5 and MMP - 2 and its relationship with pa- tients' condition and prognosis were analyzed. Results The positive rate of ARK5 and MMP - 2 was 67. 9% (72/106) and 65.1% (69/106) respectively in SCLC group and 5.0% (2/40) and 7.5% (3/40) respectively in control group, and the difference was statistically significant (X2 were 46. 004 and 38. 540, P 〈 0.05 ) . The positive rate of ARK5 and MMP - 2 was correlated with tumor size, differentiation degree, invasion depth, lymph node metastasis and tumor stage (P 〈 0.05 ). Spearman correlation analysis showed that there was positive correlation between the expressions of ARK5 and MMP - 2 ( r = 0. 878, P 〈 0. 001 ) . Kaplan -Meier survival analysis showed that the five -year survival rate in patients with positive ARK5 ex- pressions was 12. 9% , significantly lower than that of patients with negative expression (χ2 = 30. 077, P 〈 0. 001 ), while the five -year survival rate in patients with positive MMP -2 expression was 16. 3%, significantly lower than that of patients with negative expression 62. 3% (χ2 = 24. 405, P 〈 0. 001 ) . Cox multi - factor analysis showed that ARK5, differentiation degree, tumor stage were independent prognostic factors for SCLC ( P 〈 0. 05) . Conclusion The expressions of ARK5 and MMP - 2 are closely related to differentiation degree, invasion depth, and lymph node metastasis and tumor stage in SCLC patients. ARK5 and MMP -2 can greatly influence patients' survival rate, and meanwhile, ARK5 is an independent risk factor for the prognosis of SCLC patients.
