目的:探讨脾虚、肾虚及脾肾两虚证在骨质疏松症发生过程的作用和特点。方法:制作大鼠脾虚、肾虚、脾肾两虚模型,分别检测各组大鼠的骨密度(BMD)以及外周血中骨保护素(OPG)、细胞核因子κΒ受体活化因子配基(RANKL)、血管活性肠肽(VIP)、胃动素(MTL)、胃泌素(GAS)的含量。结果:与假手术组相比,脾虚模型组BMD及OPG含量与之无显著差异;肾虚及脾肾两虚模型组BMD及GAS、MTL、OPG含量均显著降低(P〈0.05,P〈0.01),VIP、RANKL的含量则显著增高(P〈0.01)。脾肾两虚模型组上述指标的改变程度较之脾虚和肾虚模型组都更为显著(P〈0.05,P〈0.01)。结论:脾虚、肾虚、脾肾两虚在骨质疏松症发病过程中均有不同作用,产生作用差异的机制可能是:肾虚状态下,OPG、RANKL的改变较为明显;而脾虚状态与VIP、GAS、MTL的改变更为相关,脾肾两虚状态下,上述指标均会发生更为显著的变化。
Objective:To investigate the roles and characteristics of syndrome of spleen deficiency,kidney deficiency,spleen and kidney deficiency in the occurrence of osteoporosis.Methods:The rat models with syndrome of spleen deficiency,kidney deficiency and deficiency of spleen and kidney were established,and the contents of BMD and OPG,RANKL,VIP,MTL and GAS in peripheral blood of rats in each group were tested respectively.Results:The difference in contents of BMD and OPG of rats between sham operation group and syndrome of spleen deficiency group was not significant.Compared with sham operation group,the contents of BMD,GAS,MTL and OPG of rats in syndrome of kidney deficiency group and syndrome of deficiency of spleen and kidney group were decreased significantly(P〈0.05,P〈0.01),and the contents of VIP and RANKL of rats in these two groups were increased significantly(P〈0.01).The changes of the above indexes of rats in syndrome of deficiency of spleen and kidney group were more significant than that of rats in syndrome of spleen deficiency group and syndrome of kidney deficiency group.Conclusion:Spleen deficiency,kidney deficiency and deficiency of spleen and kidney play different roles in the occurrence of osteoporosis,and the mechanisms of the differences in roles among the three syndromes might be summarized as follows:the change of OPG and RANKL are more obvious in the condition of kidney deficiency;the condition of spleen deficiency is correlated with the changes of VIP,GAS and MTL;in the condition ofdeficiency of spleen and kidney,the above indexes are all changed more remarkably.
