目的探讨不同晶体液对体外红细胞流变学特性的影响。方法颈静脉采集兔血[21份,(7~8)ml/份],离心并调节红细胞比容至40%;将血液分别与生理盐水(NS)、乳酸林格液(LR)和醋酸林格液(Art)按1:2、1:1、2:1混合(n=7),同时设立空白对照组(不与晶体液混合,其余操作同晶体液组)。37℃水浴1h,再次调节红细胞比容至40%,测定红细胞的变形性和聚集性。结果3种晶体液不同混合比例各组之间的变形指数和聚集指数均没有明显差异(P〉0.05)。兔血液与晶体液体积比为1:2时,红细胞聚集峰值时间NS组(2.1±0.6)s明显高于LR组(1.0±0.3)和对照组(1.3±0.4)(P〈0.05),NS、LR和AR组红细胞聚集幅度(12.0±2.0、15.6±3.2和14.9±4.6)明显低于对照组(25.1±6.5)。兔血液与晶体液体积比为1:1时,各组之间红细胞聚集峰值时间差异不大(P〉0.05),NS和AR组聚集幅度(12.9±4.1和13.9±4.1)明显低于对照组(21.3±6.4)(P〈0.05)。兔血液与晶体液体积比为2:1时,各组之间红细胞聚集峰值时间没有明显差异(P〉0.05),NS组聚集幅度(9.5±2.2)明显低于对照组(14.6±1.7)(P〈0.05)。结论晶体液种类和用量不影响红细胞变形性,但可通过改变红细胞聚集的峰值时间和聚集幅度改变红细胞聚集性;所考察的3种晶体液降低红细胞聚集性的能力由强到弱依次为NS、AR和LR。
Objective To investigate the in vitro effect of different crystaUoids on erythrocyte rheology. Methods Whole blood was obtained from the jugular veins of 21 rabbits,7 ~ 8 ml each. The samples were centrifuged and the hema- tocrit was adjusted to 40%. The samples were diluted with NS, LR and AR at the volume ratio of rabbit blood to crystalloid of 1:2,1:1 and 2:1 ,respectively. Samples from the control rabbits were treated by the same method except dilution with crystalloid fluids. All samples were incubated for 1 h at 37℃. The hematocrit of all samples was again adjusted to 40% be- fore the measurements of erythrocyte deformability and aggregation. Results Erythrocyte elongation index (EI) and aggre- gation index (AI) in all groups were not significantly different(P 〉0.05). When the volume ratio of rabbit blood to crys- talloids was 1: 2,Top time increased significandy in the NS group (2. 1 ±0.6) compared with the LR( 1.0 ±0.3) and control groups ( 1. 3 ± 0.4) ( P 〈 0.05 ). The amplitude of RBC aggregation was significantly lower in the NS, LR, AR groups ( 12.0 ± 2.0,15.6 ± 3.2,14.9 ± 4.6) compared with control group (25.1 ± 6.5 ). When the volume ratio was 1 : 1, Top time in all groups was not significantly different(P 〉0.05). The Amplitude of RBC aggreganon was significantly lower in the NS and AR groups(12.9 ±4.1 and 13.9 ±4.1) compared with the control group(21.3 ±6.4) (P 〈0.05). When the volume ratio was 2:1 ,Top time in all groups was not significantly different(P 〉 0.05). The Amplitude of RBC aggregation was significantly lower in the NS group(9.5 ± 2.2 ) compared with the control group ( 14.6± 1.7 ) (P 〈 0.05 ). Conclusion Crystalloids have no effect on RBC deformability, but will affect RBC aggregation by changing Top time and Amplitude. It can be inferred that NS has the maximum ability of decreasing RBC aggregation,followed by AR and then LR.
