目的 研究外耳道结构性病变对耳部传声的影响,及其在听力重建中的意义。方法 根据健康志愿者耳部结构的CT扫描资料,建立包括外耳道、中耳和内耳耳蜗结构的三维有限元模型,并在此基础上修改模型,建立结构性病变外耳道结构有限元模型。对声音在耳部结构的传递进行数值模拟研究,得到正常和病态结构外耳道在声音的传递中的作用的差异。结果正常外耳道结构对传入的声音有一定的增幅作用,而病态的外耳道结构则会降低这种增幅作用,鼓膜脐部和镫骨底板的位移会减小,减小的程度取决于外耳道结构的狭窄程度。结论 外耳道结构性病变会对耳部传声造成影响,改变外耳道对声音的增幅作用。该研究结果可以为外耳道再造与听力重建手术提供术前临床数值评估依据。
Objective To report a study on effects of external acoustic meatus structural lesions on sound transmission and its potential clinical significance. Methods A three dimensional finite element model of human ear was constructed in- cluding the external acoustic meatus, middle ear and cochlea using CT scans from normal volunteers. The model was then modified to generate finite element models of external acoustic meatus structural lesions. Numerical simulation was conduct- ed to obtain functions of sound transmission through health external acoustic meatus and through external acoustic meatus with structural lesions. Results Sound pressure was amplified in a health external acoustic meatus and the amplification was reduced in a diseased external acoustic meatus, leading to decreased tympanic membrane and stapes footplate displacements. The degree of reduction was correlated to the degree of stenosis of the external acoustic meatus. Conclusions Struc tural lesions of the external acoustic meatus can affect sound transmission and impair sound pressure augmentation. This study may provide a numerical assessment during preoperative evaluation for external acoustic meatus reconstruction and hearing restoration procedures.
