目的探讨应用自体筋膜加脂肪混合物，对周围性单侧声带麻痹引起声门闭合不良患者，实施声带注射填充术的治疗效果。方法全身麻醉支撑喉镜下，应用特制Brunning高压注射器，对12例周围性单侧声带麻痹患者，取自体筋膜加脂肪混合物注入患侧声带外侧的声门旁间隙，使声带游离缘内移至越过中线1．0～1．5mm。结果11例首次注射有效，1例重复注射有效。随访7～33个月，平均13个月。发现3个月内注射侧声带体积略渐回缩，3个月后声带体积及声门形态基本维持稳定，嗓音质量明显改善。无明显不良反应。结论自体筋膜加脂肪混合物是种较理想的注射填充材料，声带注射填充术简单、疗效确切、微创、安全及无明显不良反应，是治疗单侧声带麻痹引起声门闭合不良的理想术式。

英文摘要：

Objective To investigate the therapeutic effect of vocal injection and augmentation with autogenous fascia mixed with fat on glottic insufficiency caused by peripheral unilateral vocal fold paralysis. Methods Twenty-one patients with unilateral vocal fold paralysis were enrolled in the study. Autogenous fascia and fat was extracted and mixed, then was injected into the side gap of vocal cords with special Brunning power injector under laryngofibroscope, which insulted in the free margin of vocal fold being shifted acrossing the middle line by 1.0 to 1.5 mm. Results Eleven cases were cured with one injection and 1 case with duplicate injection. All patients were followed up for 7 to 33 months with an average of 13 months. The volume of the surgical vocal cord retracted slightly within 3 months after the in- jection. The volume of the vocal cord and the glottic morphology maintained stable and the voice quality relieved obviously 3 months later. No significant side effect was observed. Conclusions Mixture of autogenous fascia with fat was a better kind of injection material. The vocal injection and augmentation was a better method for the treatment of glottic insufficiency caused by peripheral unilateral vocal fold paralysis with simple injection, accurate therapeutic effect, mini- real invasion, safety and no obvious side effect.
