目的比较乳腺血管内皮生长因子(VEGF)强阳性与阴性VEGF的浸润性导管癌的钼靶X线征象(BI—RADS标准)。方法回顾性分析VEGF(+++)组25例与阴性VEGF(-)组24例共49例浸润性导管癌患者钼靶X线资料并采用两独立样本t检验或两独立样本秩和检验进行单因素分析计算等级相关系数,比较包括腺体分型、侧别、象限、肿块形态、边缘、密度、大小、钙化形态、钙化分布、有无结构扭曲、非对称性致密、腋下淋巴结改变、皮肤改变、乳头改变、病理淋巴结转移及分级、年龄差异是否有统计学意义。结果(1)不同VEGF表达状态组与年龄呈中度负相关(r=-0.314,P=0.028),与乳头改变呈低度正相关(r=0.287,P=0.048)。余未见统计学差异。(2)VEGF(+++)组较VEGF(-)年龄[(48.3±9.9)岁郴(54.1±10.3)岁,t=-2.016,P=0.049]更小,乳头改变(Z=1.965,P=0.049)更常见,余差异无统计学意义。结论采用BI—RADS为标准,钼靶X线征象浸润性导管癌VEGF(+++)与VEGF(-)征象较为类似,钼靶检查对VEGF强阳性表达预测价值有限。
Objective To compare the mammographic X - signs in invasive ductal carcinomas (IDC) with mammary gland VEGF ( + + + ) and VEGF ( - ). Methods The retrospective analysis was carried out on 24 IDC cases with VEGF ( + + + ) and 25 ICD cases with negative VEGF. The T - test, two independent sample order and t single factor analysis were carried out for analysis. The age, the gland minute, gland side, gland quadrant, tumors shape, edge, tumor density, tumor minor axis, tumor major axis, tumor length, calcification shape, calcification distribution, structure distortion, armpit lymph node change, skin property, nipple, pathological lymph node shift, the pathological graduation difference were analyzed. Results The VEGF was significantly negatively correlated with the age of patients (r = -0. 314, P =0. 028) , while positively correlated with nipple changes (r =0. 287, P =0. 048). Furthermore, the age of VEGF ( + + + ) group was significantly elder than that of VEGF( - ) group (48.3 +9.9 vs 54. 1 + 10. 3, t = -2. 016, P = 0. 049 ) ; with significantly more frequency of nipple changes ( Z = 1. 965, P = 0. 049 ). Conclusion The X - sign is similar in both strong positive and negative VEGF ICD patients, suggesting limited predictive significance.
